摘 要:目的分析血清白脂素(Asprosin)、巨噬细胞炎症蛋白-1β(MIP-1β)、25-羟维生素D3[25-(OH)D3]水平与2型糖尿病并发骨质疏松的相关性。方法回顾性分析2022年1月~2023年12月复旦大学附属闵行医院收治的110例2型糖尿病患者的临床资料,按是否并发骨质疏松分为并发组和未并发组。比较两组患者一般资料及血清Asprosin、MIP-1β、25-(OH)D3水平;使用logistic回归分析2型糖尿病骨质疏松的危险因素;采用受试者工作特征曲线(ROC)检测预测价值。结果并发组患者48例,未并发组患者62例。两组患者年龄、糖尿病病程、血清Asprosin、MIP-1β、25-(OH)D3水平比较,差异均有统计学意义(t=4.749、7.629、11.594、5.214、11.655,P<0.05)。logistic分析显示,年龄(OR=1.508,95%CI=1.078~2.110)、糖尿病病程(OR=6.170,95%CI=1.341~28.398)、血清Asprosin(OR=1.079,95%CI=1.012~1.150)、MIP-1β(OR=1.021,95%CI=1.002~1.040)、25-(OH)D3(OR=0.461,95%CI=0.246~0.862)均是影响2型糖尿病患者并发骨质疏松的独立影响因素(P<0.05)。血清Asprosin、MIP-1β、25-(OH)D3单独检测2型糖尿病并发骨质疏松的发生的ROC的曲线下面积(AUC)分别为0.711、0.756、0.821,三者联合检测的ROC的AUC为0.920,高于单独检测(P<0.05)。结论血清Asprosin、MIP-1β、25-(OH)D3水平与2型糖尿病并发骨质疏松具有相关性,三者联合检测可为2型糖尿病并发骨质疏松的预防和早期诊断提供方向,为2型糖尿病并发骨质疏松症的研究提供一定的理论依据。Objective To analyze the correlation between serum Asprosin,Macrophage Inflammatory Protein-1β(MIP-1β),and 25-hydroxyvitamin D3[25-(OH)D3]levels and type 2 diabetes mellitus complicated with osteoporosis.Methods The clinical data of 110 patients with type 2 diabetes mellitus admitted to Minhang Hospital of Fudan University from January 2022 to December 2023 were retrospectively analyzed and divided into concurrent and uncomplicated groups according to whether they were complicated with osteoporosis.General information and serum Asprosin,MIP-1β,25-(OH)D3 levels were compared between the two study groups,and risk factors for type 2 diabetes mellitus complicating osteoporosis were analyzed using logistic regression,and the predictive value was detected by using receiver operating characteristic curve(ROC).Results There were 48 patients in the complicated group and 62 patients in the uncomplicated group.Comparison of age,diabetes duration,serum Asprosin,MIP-1β,and 25-(OH)D3 levels between the two groups showed statistically significant differences(t=4.749,7.629,11.594,5.214,11.655,P<0.05).Logistic analysis showed that age(OR=1.508,95%CI=1.078~2.110),diabetes duration(OR=6.170,95%CI=1.341~28.398),serum Asprosin(OR=1.079,95%CI=1.012~1.150),MIP-1β(OR=1.021,95%CI=1.002~1.040),and 25-(OH)D3(OR=0.461,95%CI=0.246~0.862)were independent influences affecting the complication of osteoporosis in patients with type 2 diabetes mellitus(P<0.05).The area under the curve(AUC)of the ROC of serum Asprosin,MIP-1β,and 25-(OH)D3 alone for the occurrence of type 2 diabetes mellitus complicating osteoporosis was 0.711,0.756,and 0.821,respectively,and the AUC of the ROC of the combination of the three was 0.920,which was higher than that of the test alone(P<0.05).Conclusion Serum Asprosin,MIP-1βand 25-(OH)D3 levels are correlated with type 2 diabetes complicated with osteoporosis.The combined detection of the three can provide a direction for the prevention and early diagnosis of type 2 diabetes complicated with osteoporosis,and prov
