摘 要:目的探讨联勤保障部队第九四○医院泌尿外科中心单一病例群中慢性前列腺炎患者的前列腺分泌物细菌种类及其抗药性分布,旨在为标准化临床治疗与患者的个性化医疗决策提供科学依据。方法本研究回顾性纳入了2022年1月至2023年8月期间在联勤保障部队第九四○医院泌尿外科中心接受诊断并完成前列腺分泌物培养检查的慢性前列腺炎成年男性患者共500例。样本收集后即刻送至实验室,采用血琼脂平板和中国蓝琼脂平板进行标准培养。通过法国梅里埃VITEK 2 Compact自动微生物分析系统对分离纯化的菌落进行分类鉴定,并依据美国临床及实验室标准化学会的指南完成抗生素敏感性检测。结果在样本中,457例前列腺分泌物细菌培养结果为阳性,占总体的91.4%。其中,以革兰氏阳性球菌的检出率最高,达79.4%。患者感染细菌数量级别分布显示,极少量菌群占8.1%,少量菌群占74.0%,中等量菌群占13.3%,而大量菌群仅占4.6%。在对4种主导致病细菌(包括溶血性葡萄球菌、粪肠球菌、人类葡萄球菌、草绿色链球菌)进行抗药性趋势分析时发现对替加环素、利奈唑胺、万古霉素、氨苄西林等药物显示出较高的敏感性;而对红霉素、青霉素、环丙沙星、左氧氟沙星等药物则表现出较高的抗药性。结论本中心慢性前列腺炎患者前列腺分泌物的细菌培养阳性率高,以革兰氏阳性细菌为主导,且多数患者细菌数量属于少量级别,现有常用抗生素的抗药现象普遍存在。Objective To investigate the bacterial categories and drug resistance patterns in the prostatic secretions of patients with chronic prostatitis in a single case group of the Department of Urology,the 940th Hospital of Joint Logistics Support Force of Chinese PLA,and to provide a scientific basis for standardized clinical treatment and personalized medical decision-making for patients.Methods This study retrospectively included 500 adult male patients diagnosed with chronic prostatitis and who completed prostatic secretion culture examination at the Department of Urology,the 940th Hospital of Joint Logistics Support Force of Chinese PLA from January 2022 to August 2023.After collection,samples were immediately transported to the laboratory for standard cultivation on blood agar plates and China blue agar plates.Isolated and purified bacterial colonies were classified and identified using the French Bio Mérieux VITEK 2 Compact automatic microbial analysis system.The antibiotic susceptibility test was performed according to the guidelines of the Clinical and Laboratory Standards Institute(CLSI)of the United States.Results A total of 457 cases(91.4%)showed positive bacterial culture result in prostatic secretions.Among them,gram-positive cocci were the most frequently detected,constituting 79.4% of the bacteriological findings.The distribution of bacterial quantities infected by patients showed that,minimal quantities bacteria accounted for 8.1%of cases,small quantities bacteria accounted for 74.0%,moderate quantities bacteria accounted for 13.3%,and large quantities bacteria accounted for only 4.6%.In the resistance trend analysis of the four main pathogens(including hemolytic staphylococcus,enterococcus faecium,staphylococcus epidermidis,and streptococcus viridans),it was discovered that higher sensitivity was observed for drugs such as tigecycline,linezolid,vancomycin,and ampicillin,whereas higher resistance was noted for erythromycin,penicillin,ciprofloxacin,and levofloxacin.Conclusions The rate of positive bact
