摘　　要：手足皮肤反应是接受抗肿瘤治疗过程中使用靶向药物后常见的不良反应,严重降低患者生活质量,甚至导致药物减量或中断,影响抗肿瘤疗效。西医学对症治疗效果欠佳,中医药防治手足皮肤反应收效良好。吴煜教授临证经验丰富,基于“火郁发之”理论治疗靶向药物相关手足皮肤反应,归纳其病机以“热毒、湿、瘀交阻为标,气阴亏虚为本”,临床可辨证分型为热毒蕴结型、湿热瘀阻型、阴虚血燥型,分别清热解毒、透邪外出以泻火,祛湿通瘀、凉血活血以散火,补虚扶正、滋阴养血以生肌,临床效果满意,并附验案一则以证其效。Hand-foot skin reaction(HFSR)is a common adverse reaction experienced by patients undergoing anti-cancer treatment with targeted drugs,which significantly reduces the quality of life and may lead to dosage reduction or discontinuation of the medication,thereby affecting the effectiveness of cancer treatment.Modern medical symptomatic treatments are often inadequate,while Traditional Chinese Medicine(TCM)has shown good results in preventing and treating HFSR.Professor Wu Yu,with his extensive clinical experience,treats targeted drug-related HFSR based on the theory of“Dispersing the fire that is stagnated.”He identifies that the pathogenesis is that“Combining and impeding of heat-toxin,dampness,and stasis are the superficial reason,and qi and yin deficiency is the root reason”.Clinically,it can be differentiated into the types of:heat-toxin accumulation,damp-heat stasis,and yin deficiency with wind-dryness.The therapeutic approach is designed to“clear heat and detoxify,and expel pathogens to purge fire,”“eliminate dampness and unblock the stasis,cool blood and activate blood to disperse fire,”and“tonify deficiency and reinforce the healthy qi,nnourish yin and nourish blood to regenerate tissues.”The clinical outcomes have been satisfactory,and a case study is provided to demonstrate the effectiveness.
