注射用醋酸亮丙瑞林微球联合腹腔镜手术对子宫内膜异位症患者的影响

摘　　要：目的:探讨注射用醋酸亮丙瑞林微球联合腹腔镜手术对子宫内膜异位症(EMS)患者的影响。方法:选取2021年1月—2023年9月北京怀柔医院收治的86例EMS患者。根据随机数表法将其分为研究组(n=43)与对照组(n=43)。两组均接受腹腔镜手术,对照组术后给予去氧孕烯炔雌醇片,研究组术后给予注射用醋酸亮丙瑞林微球。比较两组临床疗效,治疗前后血清学指标、卵巢功能、疼痛程度,不良反应。结果:研究组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,两组基质蛋白酶-2(MMP-2)、血管内皮生长因子(VEGF)水平均降低,研究组MMP-2、VEGF水平均低于对照组,差异有统计学意义(P<0.05)。治疗后,两组卵巢中窦卵泡数量降低,卵巢体积减小,研究组卵巢中窦卵泡数量多于对照组,卵巢体积大于对照组,差异有统计学意义(P<0.05)。治疗后,两组痛经、性交痛视觉模拟评分法(VAS)评分均降低,研究组痛经、性交痛VAS评分均低于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:注射用醋酸亮丙瑞林微球联合腹腔镜手术应用于EMS患者中,能够提高临床疗效,改善血清学指标、卵巢功能,降低疼痛程度,具有较高安全性。Objective:To explore the effect of Leuprelin Acetate Microspheres for Injection combined with laparoscopic surgery on patients with endometriosis(EMS).Method:A total of 86 EMS patients admitted to Beijing Huairou Hospital from January 2021 to September 2023 were selected.They were divided into study group(n=43)and control group(n=43)according to random number table method.Both groups received laparoscopic surgery.The control group was given Desogestrel and Ethinylestradiol Tablets,and the study group was given Leuprelin Acetate Microspheres for Injection.The clinical efficacy,serological indexes,ovarian function,pain degree before and after treatment,adverse reactions were compared between the two groups.Result:The total effective rate of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of matrix protease-2(MMP-2)and vascular endothelial growth factor(VEGF)were decreased in both groups,and the levels of MMP-2 and VEGF in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the number of follicles in the central ovarian sinus decreased and the ovarian volume decreased in the two groups,the number of sinus follicle in the ovarian in the study group was more than that in the control group,and the ovarian volume was greater than that in the control group,and the differences were statistically significant(P<0.05).After treatment,the visual analogue scale(VAS)scores of dysmenorrhea and coital pain were decreased in both groups,and the VAS scores of dysmenorrhea and coital pain in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The application of Leuprelin Acetate Microspheres for Injection combined with laparoscopic surgery in EMS patients can improve clinical effic
