基于道术结合理论研究中医药改善直肠脱垂术后排便功能的疗效价值分析

摘　　要：目的分析探讨在道术结合理论指导下,采用立体四重术式联合中医药治疗直肠脱垂,患者术后排便功能的改善与疗效。方法选择符合纳入标准的2019年9月—2021年3月期间在河北省中医院住院的直肠脱垂患者36例,以应用立体四重术式治疗并且术后30 d服用中药方剂结肠舒为方案,术前、术后30 d、术后90 d做肛管直肠测压(anorectal manometry,ARM)、球囊逼出试验(balloon expulsion test,BET)、Wexner肛门失禁评分、Wexner便秘评分,观察术前、术后30 d、术后90 d各项指标,并进行自身前后对照。结果手术后的肛管静息压(anal resting pressure,ARP)、肛管最大收缩压(anal maximal contraction pressure,AMCP)显著高于手术前,差异有统计学意义(P<0.05);手术后便秘、肛门失禁评分、BET时间显著低于手术前,差异有统计学意义(P<0.05)。结论道术结合理论指导下,立体四重术式联合中医药治疗直肠脱垂疗效确切,患者术后排便功能正常与疗效成正比。Objective To analyze the improvement and efficacy of postoperative defecation in patients with rectal prolapse by using stereoscopic quadruple surgery combined with traditional Chinese medicine under the guidance of the theory of combination of tract and surgery.Methods A total of 36 patients with rectal prolapse who were hospitalized in Hebei Provincial Hospital of Traditional Chinese Medicine from September 2019 to March 2021 who met the inclusion criteria were selected.Before surgery,30 and 90 days after surgery,anorectal manometry(ARM),balloon expulsion test(BET),Wexner anal incontinence score and Wexner constipation score were observed.Each index before surgery and 30 and 90 days after surgery were observed and compared.Results The values of anal resting pressure(ARP)and anal maximal contraction pressure(AMCP)after the surgery were significantly higher than those before the surgery,and the difference was statistically significant(P<0.05).The scores of postoperative constipation and anus incontinence score and BET were significantly lower than those before surgery,and the difference was statistically significant(P<0.05).Conclusion Under the guidance of the theory of combination of tract and surgery,stereoscopic quadruple surgery in the treatment of rectal prolapse has definite curative effect.The normal defecation function of patients after surgery is directly proportional to the therapeutic effect.
