摘　　要：目的 探究多间隙立体引流术对多间隙高位肛周脓肿患者肛门功能及血清激活素A(ACTA)、碱性成纤维细胞生长因子(bFGF)水平的影响。方法 选择2022年2月至2023年3月收治的90例多间隙高位肛周脓肿患者为研究对象,以随机数字表法将其分为对照组、观察组,每组45例。对照组接受传统切开挂线术治疗,观察组给予多间隙立体引流术治疗。比较两组的治疗效果。结果 出院时,观察组的肛管静息压、肛管舒张压、肛管最大收缩压均高于对照组,差异具有统计学意义(P<0.05)。出院时,观察组的ACTA水平低于对照组,bFGF水平高于对照组,差异具有统计学意义(P<0.05)。术后24 h,观察组的前列腺素E2(PGE2)、多巴胺(DA)水平及视觉模拟评分法(VAS)评分低于对照组,差异具有统计学意义(P<0.05)。观察组的手术时间、创面愈合时间、住院时间短于对照组,术中出血量少于对照组(P<0.05)。结论 多间隙立体引流术治疗多间隙高位肛周脓肿可减少术中出血量,调节ACTA、bFGF水平,减轻疼痛应激反应,促进创面愈合及肛门功能恢复,缩短患者住院时间,值得推广。Objective To investigate the effects of multi-gap stereoscopic drainage on anal function and serum activin A(ACTA)and basic fibroblast growth factor(bFGF)levels in patients with multi-gap high perianal abscess.Methods A total of 90 patients with multi-gap high perianal abscess admitted from February 2022 to March 2023 were selected as the research objects.The patients were divided into control group and observation group by random number table method,with 45 cases in each group.The control group was treated with traditional incision and thread-drawing,and the observation group was treated with multi-gap stereoscopic drainage.The therapeutic effects of the two groups were compared.Results At discharge,the anal resting pressure,anal diastolic pressure and anal maximum systolic pressure in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).At discharge,the level of ACTA in the observation group was lower than that in the control group,the level of bFGF was higher than that in the control group,and the differences were statistically significant(P<0.05).At 24 h after operation,the levels of prostaglandin E2(PGE2),dopamine(DA)and Visual Analogue Score(VAS)score in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The operation time,wound healing time and hospitalization time of the observation group were shorter than those of the control group,and the intraoperative blood loss was less than that of the control group(P<0.05).Conclusion Multi-gap stereoscopic drainage in the treatment of multi-gap high perianal abscess can reduce intraoperative blood loss,regulate the levels of ACTA and bFGF,alleviate pain stress response,promote wound healing and anal function recovery,and shorten hospitalization time of patients,which is worth promoting.
