基于德尔菲法的药品集采效果评价指标体系构建

摘　　要：目的:以政策目标为导向,从药品集采利益相关者角度入手,构建一整套完整的药品集中带量采购效果评价指标体系。方法:首先采用文献研究法和专家访谈法初步构建评价指标体系,然后利用德尔菲法确定评价指标;最后采用层次分析法确定各指标权重。结果:经过专家函询第一轮问卷回收率为100%,第二轮为88%,表明专家积极性较高;权威系数为0.856,表明专家权威程度较高;专家协调系数大于0.4且显著性水平P<0.001,表明本研究体系较为可靠。最终构建的评价指标体系包含一级指标4个,二级指标11个,三级指标30个,指标相应的权重为医保方面(0.2393)、医疗方面(0.3261)、医药方面(0.2420)、患者回应(0.1926)。结论:指标体系构建过程科学严谨,专家代表性、积极性、权威性、协调性均较好,可作为其他地区药品集中带量采购效果评价的参考指南。Objective:Guided by policy objectives,starting from the perspective of stakeholders in centralized drug procurement,to construct a complete set of evaluation indicators for the effectiveness of centralized drug procurement policies with quantity.Methods:Firstly,a preliminary evaluation index system was constructed by combining literature research and expert consultation;then using the Delphi method to determine the evaluation indicators.Finally,the Analytical Hierarchy Process was used to calculate weights of evaluation index system.Results:After two rounds of expert inquiry,the questionnaire response rates were 100%and 88%,respectively,indicating a high level of expert enthusiasm.The authority coefficient is 0.856,indicating that experts had a high level;The expert coordination coefficient is better than 0.4 and the level of significance is P<0.001,indicating that the research system is relatively reliable.The final evaluation index system includes 5 targets in grade 1,11 in grade 2 and 30 in grade 3,with corresponding weights for medical insurance(0.2393),healthcare(0.3261),medicine(0.2420),and patient response(0.1926).Conclusion:The construction process of the indicator system is scientifically rigorous,with good expert representativeness,enthusiasm,authority,and coordination.It can serve as a reference guide for evaluating the effectiveness of centralized and quantity based drug procurement in other regions.
