摘 要:目的比较丙泊酚复合不同浓度的地佐辛在无痛结肠镜下息肉治疗中的应用效果。方法选择2018年5月—2023年12月中国建筑第三工程局武汉中心医院收治的100例无痛结肠镜下息肉切除术患者作为研究对象,随机分为低浓度组31例、中浓度组35例及高浓度组34例,分别将0.05 mg/kg、0.1 mg/kg、0.2 mg/kg地佐辛用生理盐水稀释至5 mL进行静脉注射,之后静脉泵注2.0 mg/kg丙泊酚,术中维持3~5 mL/(kg·h)泵入。监测患者血流动力学和脑电双频指数(bispectral index,BIS),记录丙泊酚用量、术后苏醒时间及并发症。结果三组术中心率(heart rate,HR)、平均动脉压(mean artery pressure,MAP)、血氧饱和度(saturation of peripheral oxygen,SpO_(2))、BIS值均发生改变,差异有统计学意义(P<0.05),低浓度组MAP波动较中浓度组和高浓度组更明显,差异有统计学意义(P<0.05),三组之间的HR和SpO_(2)比较,差异无统计学意义(P>0.05)。低浓度组和高浓度组的BIS值较中浓度组下降更明显,差异有统计学意义(P<0.05)。与低浓度组比较,中浓度组和高浓度组丙泊酚用量明显减少,差异有统计学意义(P<0.05),且高浓度组少于中浓度组,差异有统计学意义(P<0.05)。三组之间的术后苏醒时间比较,差异无统计学意义(P>0.05)。低浓度组总的并发症发生率为29.03%,中浓度组为28.57%,高浓度组为26.47%,三组比较,差异无统计学意义(P>0.05)。结论丙泊酚复合0.1 mg/kg地佐辛用于无痛结肠镜下息肉治疗术是一种具有安全效应的麻醉策略。Objective To compare the application effect of propofol combined with different concentrations of diazoxide in the treatment of polyp under painless colonoscopy.Methods A total of 100 colon polyp patients underwent painless colonoscopy polypectomy admitted to Wuhan Central Hospital of the Third Engineering Bureau of China Construction from May 2018 to December 2023 were selected for the study,and were randomly divided into 31 cases in the low concentration group,35 cases in the medium concentration group and 34 cases in the high concentration group.The 0.05 mg/kg,0.1 mg/kg,0.2 mg/kg dizocin was diluted with saline to 5 mL for intravenous injection,followed by intravenous pumping of 2.0 mg/kg propofol,and intraoperative pumping of 3~5 mL/(kg·h)was maintained.The patients’hemodynamics and EEG bispectral index(BIS)values were monitored,and the propofol dosage,postoperative awakening time and complications were recorded.Results Heart rate(HR),mean artery pressure(MAP),saturation of peripheral oxygen(SpO_(2)),and BIS values were significantly changed in the three groups(P<0.05).The fluctuation in the low concentration group was more obvious than those of the medium concentration group and the high concentration group(P<0.05).The HR and SpO_(2) among the three groups showed no significant difference(P>0.05).The BIS values in the low concentration group and the high concentration group significantly decreased more than those of the medium concentration group (P <0.05). Compared with the lowconcentration group, the propofol dosage in the medium concentration and high-concentration groups decreasedsignificantly (P <0.05), and the high concentration group was less than the medium concentration group (P <0.05).Comparison of postoperative awakening time among the three groups showed no significant difference (P >0.05).The total complication rate was 29.03% in the low-concentration group, 28.57% in the medium concentration group,and 26.47% in the high concentration group, and the difference was not significant (P >0.05).
