消银颗粒联合他扎罗汀倍他米松对寻常型银屑病患者的影响  

Effects of Xiaoyin Granules Combined with Tazarotene and Betamethasone on Patients with Psoriasis vulgaris

摘　　要：目的探讨消银颗粒联合他扎罗汀倍他米松对寻常型银屑病患者临床疗效及外周血CC趋化因子配体(CCL20)、白介素-17(IL-17)、转化生长因子-β1(TGF-β1)水平的影响。方法选取2021年1月至2022年6月于濮阳市中医医院进行治疗的200例寻常型银屑病患者为研究对象,按照随机数字表法将患者分为观察组与对照组,各100例。对照组患者给予他扎罗汀倍他米松治疗,观察组在对照组基础上联合口服消银颗粒治疗。对比两组患者银屑病面积和严重程度指数(PASI)评分、临床疗效、实验室指标水平变化情况、不良反应情况发生情况。结果观察组和对照组治疗前、治疗后2周、4周、8周、12周的PASI评分比较,不同时间点间PASI评分均随时间降低;观察组和对照组的PASI评分变化趋势比较,观察组降低更明显。观察组治疗有效率(86.00%)高于对照组(72.00%),差异有统计学意义(P<0.05)。两组治疗后CCL20、IL-17、TGF-β1水平均较治疗前降低,且观察组各项指标水平均低于对照组,差异有统计学意义(P<0.05);两组各不良反应发生率比较,差异无统计学意义(P>0.05)。结论对寻常型银屑病患者给予消银颗粒联合他扎罗汀倍他米松治疗可显著提升治疗疗效,降低外周血CCL20、IL-17、TGF-β1水平,并且不会额外增加患者的治疗风险,值得临床应用。Objective To explore the clinical efficacy of Xiaoyin Granules combined with Tazarotene and Betamethasone on patients with psoriasis vulgaris and its effects on peripheral blood CC chemokine ligand 20(CCL20),interleukin-17(IL-17),and transforming growth factor-beta 1(TGF-β1)levels.Methods A total of 200 patients with psoriasis vulgaris treated at Puyang Traditional Chinese Medicine Hospital from January 2021 to June 2022 were selected as study subjects.The patients were randomly divided into an observation group and a control group,with 100 cases in each group.The control group received treatment with Tazarotene and Betamethasone,while the observation group received oral Xiaoyin Granules in addition to the treatment in the control group.The Psoriasis Area and Severity Index(PASI)score,clinical efficacy,laboratory indicators,and incidence of adverse reactions were compared between the two groups.Results The PASI scores of both the observation group and the control group decreased over time at 2 weeks,4 weeks,8 weeks,and 12 weeks post-treatment,with the observation group showing a more significant reduction in PASI scores compared to the control group.The treatment effective rate in the observation group(86.00%)was higher than that of the control group(72.00%),with a statistically significant difference(P<0.05).After treatment,the levels of CCL20,IL-17,and TGF-β1 decreased in both groups,and the observation group exhibited lower levels of all these indicators compared to the control group,with statistically significant differences(P<0.05).No statistically significant difference was found in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Treatment of patients with psoriasis vulgaris using Xiaoyin Granules combined with Tazarotene and Betamethasone significantly enhances therapeutic efficacy,reduces peripheral blood levels of CCL20,IL-17,and TGF-β1,and does not increase treatment risks,making it worthy of clinical application.
