摘　　要：目的:探究不同剂量阿芬太尼对乳腺肿块切除术患者心血管反应的影响。方法:选择2023年3—12月于南昌市人民医院行乳腺肿块切除术的80例患者,采取随机数字表法将其分为低剂量组(5μg/kg阿芬太尼复合丙泊酚麻醉)与高剂量组(10μg/kg阿芬太尼复合丙泊酚麻醉),各40例。比较两组麻醉诱导前(T_(0))、切皮时(T_(1))、切除乳腺标本时(T_(2))、手术结束时(T_(3))的血流动力学[平均动脉压(MAP)、心率(HR)],比较两组苏醒时间、体动症状发生率及相关并发症发生情况。结果:HR和MAP组间、时间点及交互效应比较,差异均有统计学意义(P<0.05)。T_(1)、T_(2)、T_(3),两组HR、MAP水平均低于T_(0),且高剂量组均低于低剂量组(P<0.05)。高剂量组体动症状发生率显著低于低剂量组(P<0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:选用10μg/kg剂量的阿芬太尼用于乳腺肿块切除术,患者血流动力学更加平稳,体动发生率低,心血管反应轻微。Objective:To explore the effect of different doses of Alfentanil on cardiovascular response in patients undergoing breast mass resection.Method:A total of 80 patients who underwent breast mass resection in Nanchang People's Hospital from March to December 2023 were selected and divided into low-dose group(5μg/kg Alfentanil combined with Propofol)and high-dose group(10μg/kg Alfentanil combined with Propofol)by random number table method,with 40 cases in each group.The hemodynamics[mean arterial pressure(MAP),heart rate(HR)]of the two groups were compared before anesthesia induction(T_(0)),at the time of skin resection(T_(1)),at the time of breast specimen resection(T_(2)),and at the end of surgery(T_(3)).The time of recovery,incidence of somatic symptoms and related complications between the two groups were compared.Result:The differences between HR and MAP groups,time points and interaction effects were statistically significant(P<0.05).At,T_(1),T_(2) and T_(3),the levels of HR and MAP in two groups were lower than those in T_(0),and those in high-dose group were lower than those in low-dose group(P<0.05).The incidence of motor symptoms in high-dose group was significantly lower than that in low-dose group(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:10μg/kg dose of Alfentanil for breast mass resection,patients with more stable hemodynamics,low incidence of body motion,cardiovascular reaction is mild.
