摘　　要：目的评价关节镜下肱二头肌长头腱(LHBT)转位上关节囊重建术治疗巨大肩袖损伤的临床疗效。方法去我院自2019年11月至2023年9月对24例(24肩)巨大肩袖损伤患者行关节镜下LHBT转位上关节囊重建术,临床采用Constant-Murley评分、VAS评分、美国肩肘外科(ASES)评分、加州大学洛杉矶分校(UCLA)评分进行分析,肩关节活动度检查包括前屈上举、体侧内旋、体侧外旋。结果术后平均随访时间为17个月(6~26个月),Constant-Murley评分、VAS评分、ASES评分、UCLA评分显著改善,差异有统计学意义(P<0.05),患肩关节前屈上举、体侧外旋、体侧内旋功能术后提高明显。结论关节镜下LHBT转位上关节囊重建术治疗巨大肩袖损伤,患肩功能及活动度恢复良好,临床疗效满意,近期再撕裂率低。Objective To analyze the clinical efficacy of arthroscopic superior capsular reconstruction for massive rotator cuff tear using the long head of biceps tendon(LHBT).Methods From November 2019 to September 2023,24 patients(24 shoulders)were performed on arthroscopic superior capsular reconstruction for massive rotator cuff tear using the LHBT.The clinical analysis was conducted using Constant-Murley score,VAS,American Shoulder and Elbow Surgeons(ASES)score,University of California,Los Angeles(UCLA)score.The shoulder range of motion tests included flexion and elevation,lateral internal rotation,and lateral external rotation.Results The average follow-up time after surgery was 17 months(range from 6 to 26 months),Constant-Murley score,VAS,ASES score and UCLA score showed significant improvement with statistical significance(P<0.05).The shoulder joint flexion and elevation,lateral external rotation,and lateral internal rotation functions of the patient improved significantly after surgery.Conclusion The effect of arthroscopic superior capsular reconstruction for massive rotator cuff tear using the LHBT was satisfactory,the function and mobility of the affected shoulder had good recovery,the re-tearing rate was low.
