摘 要:目的探讨超级显微外科技术(super microsurgical technique,SMT)在吻合指腹静脉的指尖再植中的应用效果。方法自2018年1月至2023年1月我科收治指尖再植患者52例,年龄18~58岁,根据手术方式不同分为对照组(C组)27例和SMT组25例。C组实施常规显微外科手术,术中再植指尖指腹行放血疗法;SMT组采用SMT,术中吻合再植指尖的指腹静脉。比较两组再植成活率;术后6个月随访记录两组再植指尖萎缩率、指甲长度、感觉恢复程度、血运情况和末端关节活动度。结果SMT组成活率为96.00%,高于C组74.07%(P<0.05)。SMT组再植指尖萎缩率低于C组(P<0.05),指甲长度、感觉恢复程度、血运情况均优于C组(P<0.05)。SMT组末端关节活动度优良率为100.00%,高于C组的81.48%(P<0.05)。结论SMT吻合指腹静脉提高指尖再植患者的再植成活率,促进再植指尖功能恢复。Objective To explore the application effect of super microsurgical technique(SMT)in fingertip replantation with anastomosing the digital vein.Methods From January 2018 to January 2023,52 patients with fingertip replantation were treated,aged 18 to 58 years old.According to different surgical methods,they were divided into a control group(Group C)of 27 cases and an SMT group of 25 cases.Group C underwent routine microsurgery and the replanted fingertip was treated with bloodletting therapy during surgery.The SMT group used SMT,which anastomosed the digital vein of the replanted fingertips during surgery.The survival rates of replantation between two groups were compared.The postoperative follow-up was conducted at 6 months after surgery to record the rate of fingertip atrophy,nail length,degree of sensory recovery,blood flow,and range of motion of the distal joints in two groups.Results The activity rate of SMT composition was 96.00%,higher than 74.07%in Group C(P<0.05).The atrophy rate of replanted fingertips in the SMT group was lower than that in Group C(P<0.05),and the length of nails,degree of sensory recovery,and blood flow were all better than those in Group C(P<0.05).The excellent rate of distal joint mobility in the SMT group was 100.00%,higher than 81.48%in Group C(P<0.05).Conclusion SMT anastomosis of the digital vein can improve the survival rate of replanted fingertips and promote the recovery of replanted fingertip function.
