姜黄凝胶联合自体富血小板凝胶治疗糖尿病足溃疡临床观察

摘　　要：目的 探讨姜黄凝胶联合自体富血小板凝胶(APG)治疗糖尿病足溃疡(DFU)的临床疗效。方法 选取2021年9月至2022年12月医院内分泌科收治的DFU患者125例,采用随机数字表法分为对照组(常规换药,32例)、CUR组(常规+姜黄凝胶换药,31例)、APG组(常规+APG换药,31例)、CUR+APG组(常规+姜黄凝胶+APG换药,31例),4组患者均予基础治疗及每天以对应方法换药,均连续治疗12 d。结果 CUR+APG组总有效率为96.77%,显著高于对照组的53.13%,CUR组的80.65%,APG组的77.42%(P <0.05),CUR组、APG组总有效率均显著高于对照组(P <0.05)。CUR+APG组患者治疗后溃疡的面积、愈合率、愈合时间,踝动脉与肱动脉收缩压的比值,以及C反应蛋白、降钙素原、红细胞沉降率、血管内皮生长因子、基质金属蛋白酶-9均显著优于其余3组(P <0.05);CUR组和APG组患者上述指标均显著优于对照组(P <0.05)。结论 姜黄凝胶联合APG治疗DFU,可显著提升疗效,改善患者溃疡状况。Objective To investigate the clinical efficacy of turmeric gel combined with autologous platelet-rich gel(APG)in the treatment of diabetic foot ulcer(DFU).Methods A total of 125 patients with DFU admitted to the Endocrinology Department of the hospital from September 2021 to December 2022 were selected and randomly divided into the control group(routine drugs,32 cases),CUR group(routine drugs+turmeric gel,31 cases),APG group(routine drugs+APG,31 cases)and CUR+APG group(routine drugs+turmeric gel+APG,31 cases).All patients were given basic treatment and daily corresponding dressing change.All groups were treated for 12 d.Results The total effective rate in the CUR+APG group was 96.77%,which was significantly higher than 53.13%in the control group,80.65%in the CUR group,and 77.42%in the APG group(P<0.05),and that in the CUR group and APG group was significantly higher than that in the control group(P<0.05).The ulcer area,healing rate,healing time,ankle-brachial index(ABI),C-reactive protein(CRP),procalcitonin(PCT),erythrocyte sedimentation rate(ESR),vascular endothelial growth factor(VEGF),and matrix metalloproteinase-9(MMP-9)in the CUR+APG group after treatment were significantly better than those in the other three groups(P<0.05);the above indicators in the CUR group and APG group were significantly better than those in the control group(P<0.05).Conclusion Curcuma gel combined with APG in the treatment of DFU can significantly improve the efficacy and ulcer healing.
