斜外侧腰椎椎间融合术在成人退变性脊柱侧凸中的应用进展

摘　　要：成人退变性脊柱侧凸(adult degenerative scoliosis,ADS)困扰着大量的老年患者,导致严重的疼痛及功能障碍,降低生活质量。当保守治疗无效,疾病严重影响患者日常生活时,手术干预是首选的办法。既往以传统的后路截骨矫形手术为主,该术式创伤大、各种并发症发生率高,可能降低手术满意度。斜外侧腰椎椎间融合术(oblique lumbar interbody fusion,OLIF)通过自然的解剖间隙进行操作,最大限度地避免对正常结构的破坏,降低对患者的手术创伤。本文对筛选适合行OLIF的ADS病例、OLIF联合新型技术在ADS诊疗中的优点及OLIF在改善矢状面平衡的优势等方面进行综述。Adult degenerative scoliosis(ADS)afflicts a large number of elderly patients,resulting in severe pain,dysfunction,and reduced quality of life.When conservative treatment is ineffective and the disease seriously affects the patient's daily life,surgical intervention is the first choice.In the past,traditional posterior osteotomy and orthopaedic surgery was the main method,which was characterized by high trauma and high incidence of various complications,which may reduce the satisfaction of surgery.Oblique lumbar interbody fusion(OLIF)operates through a natural anatomical gap to minimize damage to the normal structure and minimize surgical trauma to the patient.This article reviews the selection of ADS cases suitable for OLIF,the advantages of OLIF combined with new technology in ADS diagnosis and treatment,and the advantages of OLIF in improving sagittal balance.
