摘　　要：结直肠腺瘤是明确的癌前病变,在结直肠腺瘤-癌演化的过程中,“炎癌转化”是关键病理环节。在中医“治未病”理论的指导下,未病先防、既病防变成为阻断“炎癌转化”和防治结直肠癌癌前病变的共识和临床研究的重要课题。本文从不同层面探讨中医药防治“炎癌转化”的有效性,其作用优势体现在改善中医症状、提高生活质量等方面,其机制可能是通过抑制促癌信号激活、调节炎症反应和维持肠道微生物稳态、保护肠道屏障等方面发挥作用。本文通过阐释腺瘤-癌转化的中医病机,探讨中医药截断“炎癌转化”的疗效及优势,分析干预靶点和通路,为中医药介入结直肠癌癌前病变的管理和中西医结合协同防治提供参考。Colorectal adenoma is a definite precancerous lesion,and the“inflammation-cancer transformation”is a key pathological link in the process of colorectal adenoma-cancer evolution.Under the guidance of the theory of“preventive treatment of diseases”in TCM,prevention before disease onset and preventing the development of the occurred disease has become the consensus and the important subject of clinical research to block the“inflammationcancer transformation”and prevent the precancerous lesions of colorectal cancer.This article discussed the effectiveness of TCM in preventing and treating the“inflammation-cancer transformation”from different aspects.Its advantages are reflected in improving the symptoms of TCM and improving the quality of life,etc.Its mechanism may be related to inhibiting the activation of cancer-promoting signals,regulating inflammation,maintaining intestinal microbial homeostasis,and protecting the intestinal barrier.By explaining the pathogenesis of adenomacancer transformation in TCM,this article discussed the efficacy and advantages of TCM to block“inflammationcancer transformation”,analyzed its intervention targets and pathways,providing references for the intervention of TCM in the management of precancerous lesions of colorectal cancer and the synergistic prevention and treatment of traditional Chinese and Western medicine.
