基于多元数据挖掘探讨程宛钧辨治血精症用药规律

摘　　要：目的通过数据挖掘技术结合专家访谈探讨程宛钧教授治疗血精症的用药规律。方法收集2019年1月-2023年5月福建中医药大学附属人民医院程宛钧教授治疗血精症的门诊病历,整理归纳为医案处方数据库,运用古今医案云平台2.3.9、Excel2019、Cytoscape3.10.1及SPSS Statistics 25.0软件对数据进行分析,统计年龄分层、药物频次、药效频次、性味归经,并进行关联规则分析、系统聚类分析和因子分析,结合专家访谈得出最终结论。结果共纳入程宛钧教授治疗血精症的医案处方111首,涉及中药119味,用药总频次为1853。其中以血余炭、地榆炭频次最高;以补虚、止血、清热、活血化瘀功效为主;药性以寒、温为主,药味以苦、甘为主,多归肝经、肺经;关联规则分析得出9组对药组合,12组角药组合,4组四味药物组合;聚类分析得到新处方3首;因子分析提取公因子7个。结论程宛钧教授治疗血精症以疏肝解郁、清热利湿、活血化瘀、清热凉血为主,兼以养阴清热止血、健脾养血益气。Objective To explore Professor Cheng Wanjun’s medication law in the treatment of hemospermia through data mining techniques combined with expert interviews.Methods The outpatient medical records of Cheng Wanjun for the treatment of hematospermia in the People’s Hospital Affiliated to Fujian University of Traditional Chinese Medicine were collected from January 2019 to May 2023,which were organized and summarized into a medical prescription database.The database was statistically analyzed by using Ancient and Modern Medical Case Cloud Platform 2.3.9,Excel 2019,Cytoscape 3.10.1 and SPSS Statistics 25.0 software to statistically analyze age stratification,drug frequency,efficacy frequency,property and taste and meridian,as well as association rules analysis,systematic clustering analysis and factor analysis.Combined with expert interviews,final conclusions were achieved.Results A total of 111 medical prescriptions of Professor Cheng Wanjun for the treatment of hematospermia were included,involving 119 kinds of Chinese materia medica,with a total frequency of 1853 times.Among them,carbonized hair and processed Sanguisorbae Radix were used most frequently;most of them were used for tonifying deficiency,stopping bleeding,clearing heat and activating blood circulation and removing blood stasis;the main properties were cold and warm,and the main tastes were bitter and sweet,and most of them belonged to liver and lung meridians;the association rules analysis yielded 9 groups of medicinal pairs,12 groups of combinations with three kinds of Chinese materia medica,and 4 groups of combinations with four kinds of Chinese materia medica;the clustering analysis yielded three new prescriptions;the factor analysis extracted seven common factors.Conclusion Professor Cheng Wanjun’s treatment of hemospermia is based on soothing the liver,clearing heat and dampness,activating blood circulation,removing blood stasis,clearing heat and cooling blood,and supplemented with nourishing yin,clearing heat and stopping bleeding,strengthen
