摘 要:目的:本研究旨在评估不同重建算法对^(18)F-PSMA-1007 PET/CT在前列腺癌诊断中的半定量准确性和图像质量的影响,探讨其在提高诊断效能方面的潜力。方法:回顾性分析2022年4月至2024年4月期间,97例疑似前列腺癌患者的^(18)F-PSMA-1007 PET/CT数据。数据采用Iterative+VUE Point、3D-Reprojection、3D-FORE-FBP及3D-FOREIterative四种算法进行重建。以穿刺或手术病理结果为“金标准”,比较不同算法下的诊断效能(灵敏度、特异度、准确度)及半定量参数(SUVmax、SUVmean、SUVpeak)。结果:3D-FORE-Iterative算法在阳性率、准确度、灵敏度、特异度、阳性及阴性预测值方面均优于其他算法(P<0.05)。SUVmean、SUVmax、SUVpeak及Std在3D-FORE-Iterative算法下均低于其他算法(P<0.05)。图像质量评分显示,3D-FORE-Iterative算法在图像质量、清晰度和病变显著性方面得分最高(P<0.05)。结论:3D-FORE-Iterative算法在前列腺癌^(18)F-PSMA-1007 PET/CT诊断中显示出最高的诊断效能和图像质量,为临床提供了更精确的诊断工具。未来的研究应关注该算法在不同患者群体中的普适性和长期临床效果。Objective:This study was designed to evaluate the effects of different reconstruction algorithms on the semi-quantitative accuracy and image quality of^(18)F-PSMA-1007 PET/CT in the diagnosis of prostate cancer,and to explore its potential in improving diagnostic efficiency.Methods:We retrospectively analyzed^(18)F-PSMA-1007 PET/CT data from 97 patients with suspected prostate cancer between April 2022 and April 2024.Data is reconstructed using four algorithms:Iterative+VUE Point,3DReprojection,3D-FORE-FBP and 3D-FORE-Iterative.The diagnostic efficacy(sensitivity,specificity,accuracy)and semi-quantitative parameters(SUVmax,SUVmean,SUVpeak)under different algorithms were compared with the pathologic results of puncture or surgery as the"gold standard".Results:3DFORE-Iterative algorithm was superior to other algorithms in positive rate,accuracy,sensitivity,specificity,positive and negative predictive value(P<0.05).SUVmean,SUVmax,SUVpeak and Std were lower than other algorithms under 3D-FORE-Iterative algorithm(P<0.05).Image quality scores showed that 3D-FORE-Iterative algorithm had the highest scores in image quality,sharpness and lesion significance(P<0.05).Conclusion:3D-FORE-Iterative algorithm shows the highest diagnostic efficiency and image quality in the^(18)F-PSMA-1007 PET/CT diagnosis of prostate cancer,and provides a more accurate diagnostic tool for clinic.Future studies should focus on the universality and long-term clinical effects of the algorithm in different patient populations.
