摘 要:目的 探讨阿奇霉素联合酚妥拉明治疗支气管扩张(BE)患者的疗效及对肺功能的影响。方法 选取本院收治的BE患者90例(2021年12月至2023年12月),随机将患者分为酚妥拉明组(45例)和联用组(45例)。酚妥拉明组患者采用酚妥拉明20mg治疗,联用组患者在酚妥拉明组基础上联合阿奇霉素0.25g治疗,两组患者均连续治疗2周。观察两组患者治疗后的临床疗效、肺功能、血气指标、免疫功能、炎症因子和不良反应发生情况。结果 治疗后,联用组治疗后总有效率为95.56%(43/45),显著高于酚妥拉明组的82.22%(37/45)(P<0.05)。两者患者治疗后的第1秒用力呼气容积(FEV1)、最大肺活量(FVC)、最大通气量(MVV)水平显著升高(P<0.05),联用组显著高于酚妥拉明组(P<0.05)。治疗后,两组患者血氧分压(PaO_(2))、血氧饱和度(SaO_(2))、CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平较治疗前均显著提高,且联用组显著高于酚妥拉明组(P<0.05),而血二氧化碳分压(PaCO_(2))、白细胞介素-4(IL-4)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)水平明显降低,联用组低于酚妥拉明组(P<0.05)。两组患者的不良反应发生率差异无统计学意义(P>0.05)。结论 阿奇霉素联合酚妥拉明治疗BE患者,可以提高其临床疗效,促进肺功能恢复,改善血气指标,提高免疫功能,降低炎症反应。Objective To evaluate the efficacy of azithromycin combined with phentolamine in patients with bronchiectasis(BE) and its impact on lung function.Methods Ninety BE patients admitted to our hospital from December,2021 to December,2023 were randomly separated into a phentolamine group(45 cases) and a combination group(45 cases).The phentolamine group received treatment with phentolamine(20mg),while the combination group received azithromycin(0.25g) in addition to the phentolamine group.Both groups were treated continuously for 2 weeks.The clinical efficacy,lung function,blood gas indicators,immune function,inflammatory factors,and adverse reactions were observed and compared between these two groups.Results After treatment,the total effective rate of the combination group was 95.56%(43/45),which was significantly higher than 82.22%(37/45) of the phentolamine group(P<0.05).The levels of forced expiratory volume in the first second(FEV1),forced vital capacity(FVC),and maximum ventilation volume(MVV) of both patient groups were significantly increased(P<0.05),and the combination group was significantly higher than that of the phentolamine group(P<0.05).After treatment,the levels of blood oxygen partial pressure(PaO_2),blood oxygen saturation(SaO_2),CD3~+,CD4~+,CD4~+/CD8~+ in both groups were significantly increased compared to before treatment,and the combination group was significantly higher than that of the phentolamine group(P<0.05),while the levels of blood partial pressure of carbon dioxide(PaCO_2),interleukin-4(IL-4),tumor necrosis factor-α(TNF-α),and interleukin-6(IL-6) were significantly reduced,and the combination group was significantly lower than that of the phentolamine group(P<0.05).There was no significant difference in adverse reactions between these two groups(P>0.05).Conclusion The combination of azithromycin and phentolamine in the treatment of BE patients can improve clinical efficacy,promote lung function recovery,improve blood gas indicators,enhance immune function,and reduce inflammatory react
