摘　　要：目的 探讨不同时机机械取栓对急性缺血性脑卒中(AIS)患者发生下肢深静脉血栓(DVT)的影响。方法 收集2020年4月至2023年9月于海安市人民医院接受机械取栓治疗的122例AIS患者的临床资料,按照机械取栓时机的不同将其分为早期组(n=57,在发病6 h内接受机械取栓治疗)和晚期组(n=65,在发病6~24 h内接受机械取栓治疗),比较两组患者住院期间的主要结局指标及并发症发生率。结果 早期组患者下肢DVT发生率、患肢肌力≤2级比例均低于对照组患者(P﹤0.05)。两组患者感染率、症状性颅内出血(s ICH)发生率、偏瘫比例、术中末次造影血管良好再通率比较,差异均无统计学意义(P﹥0.05)。结论 AIS患者发病6 h内进行机械取栓可降低住院期间下肢DVT的发生风险。Objective To investigate the effect of mechanical thrombectomy at different times on deep vein thrombosis(DVT) of lower extremity in patients with acute ischemic stroke(AIS).Method Clinical data of 122 AIS patients who received mechanical thrombectomy in Hai'an People's Hospital from April 2020 to September 2023 were collected and divided into early group(n=57,mechanical thrombectomy within 6 h of onset) and late group(n=65,mechanical thrombectomy within 6 to 24 h of onset) according to different timing of mechanical thrombectomy.The main outcome indexes and complication rate during hospitalization were compared between the two groups.Result During hospitalization,the incidence of DVT in lower extremity and the proportion of muscle strength≤grade 2 of affected extremity in early group were lower than those in control group(P<0.05).There were no significant differences in infection rate,incidence of symptomatic intracranial hemorrhage(sICH),proportion of hemiplegia,and rate of good vascular recirculation during last angiography between two groups(P>0.05).Conclusion Early mechanical thrombectomy(within 6 of onset) can reduce the risk of lower extremity DVT in AIS patients during hospitalization.
