摘　　要：目的 探讨间歇性气压治疗对剖宫产术后发生下肢深静脉血栓(DVT)的预防作用。方法 收集2017年1月至2024年4月于中山市小榄人民医院接受剖宫产术的400例产妇的临床资料,按照是否接受间歇性气压治疗将其分为治疗组(n=211)和对照组(n=189),比较两组产妇剖宫产住院期间的相关指标。结果 两组产妇孕次、产次、妊娠合并症、手术时间、术中出血量、低分子肝素治疗情况、足背屈运动情况比较,差异均无统计学意义(P﹥0.05)。所有新生儿均正常生存,所有产妇住院期间共发生17例(4.3%)下肢DVT,其中,治疗组产妇下肢DVT发生率为1.9%(4/211),低于对照组的6.9%(13/189),差异有统计学意义(P﹤0.05)。出院前复查检测结果显示,治疗组产妇D-二聚体升高发生率为18.5%(39/211),低于对照组的30.2%(57/189),差异有统计学意义(P﹤0.05)。两组产妇复查凝血酶原时间、活化部分凝血活酶时间、切口感染率、切口愈合不良率比较,差异均无统计学意义(P﹥0.05)。结论 间歇性气压治疗可以安全有效降低剖宫产术后下肢DVT的发生风险,值得临床推广应用。Objective To investigate the preventive effect of intermittent barotherapy on deep vein thrombosis(DVT) of lower extremity after cesarean section.Method Clinical data of 400 women who received cesarean section in Xiaolan Hospital Affiliated to Southern Medical University from January 2017 to April 2024 were collected,they were divided into treatment group(n=211) and control group(n=189) according to whether they received intermittent barometric therapy,and the relevant indicators during hospitalization during cesarean section were compared between the two groups.Result There were no significant differences in pregnancy times,delivery times,pregnancy complications,operation time,intraoperative blood loss,low molecular weight heparin treatment and dorsiflexion movement between the two groups(P>0.05).All neonates survived normally,17 cases(4.3%) of lower extremity DVT occurred during hospitalization.Among them,the incidence of lower extremity DVT in the treatment group was 1.9%(4/211),lower than that in the control group of 6.9%(13/189),with statistical significance(P<0.05).The re-examination results before discharge showed that the incidence of D-dimer elevation in the treatment group was 18.5%(39/211),which was lower than that in the control group of 30.2%(57/189),the difference was statistically significant(P<0.05).There were no significant differences in prothrombin reexamination time,activated partial thromboplastin time,incision infection rate and incision healing rate between the two groups(P>0.05).Conclusion Intermittent pneumatic therapy can safely and effectively reduce the risk of lower extremity DVT after cesarean section,and is worthy of clinical promotion and application.
