通脉汤联合Gamma3髓内钉治疗不稳定型股骨粗隆间骨折的临床疗效

摘　　要：目的探讨通脉汤联合Gamma3髓内钉治疗不稳定型股骨粗隆间骨折的临床疗效。方法选取2019年1月至2021年12月瑞金市中医院骨科收治的78例不稳定型股骨粗隆间骨折患者作为研究对象,按照信封法随机分为对照组与观察组,每组39例。对照组给予Gam-ma3髓内钉内固定治疗,观察组在对照组基础上给予通脉汤治疗。比较两组中医症状积分、髋关节功能评分、实验室指标[血小板计数(plate-letcount,PLT)、D-二聚体(D-dimer,D-D)]及临床疗效。结果术后15d,两组肿胀、疼痛、瘀斑评分均低于术前,且观察组低于对照组,差异有统计学意义(P<0.05)。术后6个月,两组关节畸形、活动度、疼痛、功能评分均高于术前,且观察组高于对照组,差异有统计学意义(P<0.05)。术后7d,两组血清PLT、D-D水平均低于术前,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组治疗总有效率为92.31%,明显高于对照组的74.36%,差异有统计学意义(P<0.05)。结论通脉汤联合Gamma3髓内钉治疗不稳定型股骨粗隆间骨折,临床疗效显著,可缓解患者肿胀、疼痛症状,改善血液高凝状态,提高髋关节功能,值得临床推广应用。Objective To explore the clinical efficacy of Tongmai decoction combined with Gamma3 intramedullary nail in the treatment of un-stable intertrochanteric fracture of femur.Methods 78 patients with unstable intertrochanteric fracture of femur admitted to the Orthopedics depart-ment of Ruijin Traditional Chinese Medicine Hospital from January 2019 to December 2021 were selected as the study subjects,and they were divid-ed into the control group and the observation group by envelope method,with 39 cases in each group.The control group was treated with Gamma3 in-tramedullary nail internal fixation,and the observation group was treated with Tongmai decoction on the basis of the control group.The Traditional Chinese Medicine symptom scores,hip function scores,laboratory indexes(serum platelet count[PLT]and D-dimer[D-D])and clinical efficacy were compared between the two groups.Results At 15 d after operation,the scores of swelling,pain and ecchymosis of the two groups were lower than those before operation,and the observation group was lower than the control group,the differences were statistically significant(P<0.05).At 6 months after operation,the scores of joint deformity,activity,pain and function of the two groups were higher than those before operation,and the ob-servation group was higher than the control group,the differences were statistically significant(P<0.05).At 7 d after operation,the levels of serum PLT and D-D of the two groups were lower than those before operation,and the observation group was lower than the control group,the differences were statistically significant(P<0.05).The total effective rate of treatment in the observation group was 92.31%,which was higher than 74.36%in the control group,and the difference was statistically significant(P<0.05).Conclusion Tongmai decoction combined with Gamma3 intramedullary nail has significant clinical efficacy in the treatment of unstable femoral intertrochanteric fractures,it can relieve swelling and pain symptoms,im-prove blood hypercoagulability,and impro
