腹腔镜下系膜剥离法阑尾切除术对急性阑尾炎患者术后炎症反应的影响研究

摘　　要：探讨腹腔镜下系膜剥离法阑尾切除术对急性阑尾炎患者术后炎症反应的影响。以2023年1月—2024年5月盘州市人民医院收治的96例急性阑尾炎患者为研究对象,将其随机分为对照组和观察组两组,每组各48例。对照组使用常规腹腔镜阑尾切除术,观察组使用腹腔镜下系膜剥离法阑尾切除术,对比两组患者的各项手术指标、炎症指标、并发症发生率。结果表明,观察组各项手术及术后恢复指标均优于对照组(P<0.05);观察组术后1天的CRP、PCT、WBC、NEUT等指标水平均低于对照组(P<0.05);观察组感染、肠梗阻、活动性出血、腹腔粘连等并发症发生率低于对照组(P<0.05);观察组术后1天、3天的VAS评分低于对照组,手术满意度高于对照组(P<0.05)。研究发现,腹腔镜下系膜剥离法阑尾切除术对急性阑尾炎患者的疗效显著,手术创伤小,术后患者的炎症反应及疼痛感轻,术后恢复快,并发症发生率低,明显提高了患者的手术满意度。To explore the effect of laparoscopic mesentery appendectomy on postoperative inflammatory response in patients with acute appendicitis.96 patients with acute appendicitis admitted to Panzhou People’s Hospital from January 2023 to May 2024 were randomly divided into a control group and an observation group,with 48 patients in each group.The control group underwent conventional laparoscopic appendectomy,while the observation group underwent laparoscopic mesentery appendectomy.The surgical indicators,inflammatory indicators,and incidence of complications were compared between the two groups of patients.The results showed that the observation group had better surgical and postoperative recovery indicators than the control group(P<0.05);The levels of CRP,PCT,WBC,NEUT and other indicators in the observation group were lower than those in the control group one day after surgery(P<0.05);The incidence of complications such as infection,intestinal obstruction,active bleeding,and abdominal adhesions in the observation group was lower than that in the control group(P<0.05);The VAS pain scores of the observation group were lower than those of the control group on postoperative days 1 and 3,and the surgical satisfaction was higher than that of the control group(P<0.05).Research has found that laparoscopic mesentery appendectomy has significant therapeutic effects on patients with acute appendicitis,with minimal surgical trauma,mild postoperative inflammation and pain,fast postoperative recovery,low incidence of complications,and significantly improved patient satisfaction with the surgery.
