摘　　要：目的分析体外冲击波联合运动疗法对骨盆肌筋膜疼痛综合征疼痛视觉模拟评分法(VAS)及女性性功能量表(FSFI)的影响。方法选择骨盆肌筋膜疼痛综合征患者共60例,利用随机数字表法分为对照组(常规治疗,n=30)与研究组(体外冲击波+运动疗法,n=30)。对比2组患者的疼痛程度、性功能、炎症因子、疼痛介质及生长因子。结果在VAS评分与FSFI评分上,研究组优于对照组(P<0.05);在肿瘤坏死因子-α(TNF-α)与白细胞介素-18(IL-18)水平上,研究组均低于对照组(P<0.05);在5-羟色胺(5-HT)与前列腺素E2(PGE2)水平上,研究组低于对照组(P<0.05);在血清转化生长因子(TGF-β1)与胰岛素样生长因子(IGF-1)水平上,研究组高于对照组(P<0.05)。结论体外冲击波+运动疗法有利于减轻骨盆肌筋膜疼痛综合征患者的疼痛程度,改善性功能,减轻炎症反应。Objective To analyze the effects of extracorporeal shock wave combined with exercise therapy on visual analogue scale(VAS)and female sexual function scale(FSFI)in pelvic myofascial pain syndrome.Methods 60 patients with pelvic myofascial pain syndrome were enrolled and divided into control group(conventional treatment,n=30)and study group(extracorporeal shock wave+exercise therapy,n=30)by random number table method.The pain degree,sexual function,inflammatory factors,pain mediators and growth factors were compared between the two groups.Results The study group was superior to the control group in VAS and FSFI scores(P<0.05).The levels of tumor necrosis factor-α(TNF-α)and interleukin-18(IL-18)in the study group were lower than those in the control group(P<0.05).The levels of 5-hydroxytryptamine(5-HT)and prostaglandin E2(PGE2)in the study group were lower than those in the control group(P<0.05).Serum transforming growth factor(TGF-β1)and insulin-like growth factor(IGF-1)levels in the study group were higher than those in the control group(P<0.05).Conclusion Extracorporeal shock wave combined with exercise therapy can improve the pain degree and sexual function of the patients with pelvic myofascial pain syndrome,and reduce the inflammatory response.
