全切开法重睑成形术中保留部分眼轮匝肌前静脉对患者术后并发症及恢复情况的影响

摘　　要：目的探讨全切开法重睑成形术中保留部分眼轮匝肌前静脉对患者术后并发症及恢复情况的影响。方法选择30例行双侧眼睑切开的上睑下垂或上睑内翻患者为研究对象,按照随机数字表法将其分成研究组、对照组,每组15例。研究组采用全切开法重睑成形术中保留部分眼轮匝肌前静脉治疗,对照组实施未保留眼轮匝肌前静脉的常规全切开法重睑成形术治疗。比较2组患者手术相关指标、术后瘢痕情况、手术前后眼睑角度、内眦间距及并发症发生情况。结果研究组术后留院时间短于对照组(P<0.05);研究组术后1个月、术后2个月温哥华瘢痕量表(VSS)评分、患者与观测者疤痕评价量表(POSAS)评分均低于对照组(P<0.05),且术后2个月,2组VSS评分、POSAS评分均低于术后1个月(P<0.05);术后2个月,2组患者眼睑角度、内眦间距均低于术前,研究组均低于对照组(P<0.05)。结论全切开法重睑成形术中保留部分眼轮匝肌前静脉治疗可缩短术后留院时间,降低瘢痕面积,促进术后恢复,安全可靠。Objective To explore the effect of preserving part of the anterior orbicularis vein in total incision blepharoplasty on postoperative complications and recovery.Methods 30 patients with ptosis or entropion with bilateral eyelid incision were selected and divided into a study group and a control group,with 15 cases in each group,according to the randomized numerical table method.The study group underwent partial preservation of the orbicularis oculi anterior vein with total incision double blepharoplasty,and the control group underwent conventional total incision double blepharoplasty without preservation of the anterior orbicularis oculi vein.The related indicators,postoperative scarring,eyelid angle,medial canthal spacing before and after surgery and complications were compared.Results The postoperative hospital stay in the study group was shorter than that in the control group(P<0.05).The vancouver scar scale(VSS)score and the observer scar evaluation scale(POSAS)scores in the study group were lower than those in the control group at 1 month and 2 months after surgery(P<0.05).Two months after the operation,VSS score and POSAS score of two groups were lower than one month after surgery(P<0.05).Two months after surgery,eyelid angle and inner canthus distance in the two groups were lower than those before surgery(P<0.05).Conclusion The preserving part of the anterior orbicularis veins in total incision double blepharoplasty can shorten the postoperative hospital stay,reduce the scar area and promote postoperative recovery,and it is safe and reliable.
