摘　　要：目的探讨计算机断层扫描(CT)引导下背根神经节脉冲射频(DRG-PRF)联合神经阻滞治疗带状疱疹后遗神经痛(PHN)患者临床疗效及安全性。方法纳入2022年5月至2024年3月在安徽省庐江县人民医院接受治疗的96例PHN患者,使用随机数字表法分为联合治疗组和对照组各48例。联合治疗组患者给予DRG-PRF联合神经阻滞治疗,对照组给予神经阻滞治疗。比较2组PHN患者治疗前与治疗1、2、4、8周后,患者疼痛程度[视觉模拟评分(VAS)]、睡眠质量[匹兹堡睡眠质量指数(PSQI)评分]、口服镇痛药的用量以及痛觉敏感度,并观察不良反应。结果治疗1、2、4、8周后,2组患者疼痛VAS评分、PSQI评分、口服镇痛药物用量以及痛觉敏感度均显著降低(P<0.05),联合治疗组低于对照组(P<0.05);治疗期间,2组患者的不良反应发生率差异无统计学意义(P>0.05)。结论CT引导下DRG-PRF联合神经阻滞治疗PHN患者能有效减轻患者疼痛,减少镇痛药物用量,改善患者睡眠质量,安全有效。Objective To investigate the efficacy and safety of CT-guided dorsal root ganglion pulsed radiofrequency(DRG-PRF)combined with nerve block in the treatment of patients with postherpetic neuralgia(PHN).Methods 96 patients with PHN who received treatment at Lujiang County People's Hospital from May 2022 to March 2024 were enrolled in this study,and were divided into combined therapy group and control group by random number table method,with 48 cases in each group.The patients in the combined therapy group were treated with DRG-PRF combined with nerve block,while the control group was given nerve block.The pain degree[visual analogue scale(VAS)],sleep quality[pittsburgh sleep quality index(PSQD)],dosage of oral analgesics and pain sensitivity were compared between groups before treatment and after 1,2,4 and 8 weeks of treatment,and the adverse reactions were observed.Results After 1,2,4 and 8 weeks of treatment,the VAS score,PSQI score,dosage of oral analgesics and pain sensitivity of PHN patients in both groups were significantly decreased(P<0.05),and these reductions were more pronounced in combined therapy group compared to control group(P<0.05).There was no obvious difference in the overall incidence rate of adverse reactions between groups(P>0.05).Conclusion CT-guided DRG-PRF combined with nerve block can effectively relieve the pain,reduce the dosage of analgesic drugs,and improve the sleep quality in the treatment of PHN patients,demonstrating safety and feasibility.
