以连续性肢端皮炎为表现的增殖型天疱疮1例

摘　　要：患者男,76岁。口腔、腋窝、肛周反复破溃、糜烂10年,右足趾脓疱15 d余。组织病理检查示表皮呈假上皮瘤样增生,其内可见大量嗜酸粒细胞、中性粒细胞浸润伴脓肿形成,基底层上方棘层松解,裂隙形成,真皮内可见大量中性粒细胞、嗜酸粒细胞、淋巴细胞浸润,血管扩张充血。诊断:增殖型天疱疮(Hallopeau型)。予以甲泼尼龙联合环孢素治疗后皮损好转。A 76-year-old malepatient presented withrecurrent ulcerationsand erosionsin theoral cavity,axillae,and perianal region for 10 years,as well as a pustule on the right toe for more than 15 days.Histopathological examination revealed pseudo-epitheliomatous hyperplasia,with significant infiltration of eosinophils and neutrophils,accompanied by abscess formation.The acantholysis and fissure formation were observed above the basal layer.In the dermis,there was extensive infiltration of neutrophils,eosinophils,and lymphocytes,along with vascular dilatation and congestion.The patient was diagnosed as pemphigus vegetans(Hallopeau type).The patient was treated with a combination of methylprednisolone and cyclosporine,resulting in improvement of the skin lesions.
