基于“调和营卫、解郁化浊”理论探析双心病的诊疗思路

摘　　要：随着社会节奏的加快和压力的增加,心血管疾病和精神障碍,如抑郁、焦虑症,常常同时出现在同一个患者身上,这一现象被称为“双心病”。心血管疾病一直是全球导致死亡的主要原因,而精神健康问题也不断给个人及社会带来沉重的经济和社会负担。以往的研究主要关注心血管疾病或精神健康问题单独的影响,对于两者共存的状况关注较少,这导致在治疗双心病时,往往无法全面考虑患者的心脏和心理需求,进而影响治疗效果。张艳教授认为双心疾病的病机总属本虚标实,以气血营卫亏虚为本,气滞、痰浊、血瘀为标,病程迁延日久则虚实夹杂。根据张艳教授多年的经验总结,以“调和营卫、解郁化浊”为理论依据,从心、营卫、肝、脾角度认识双心疾病,将口服汤剂的内治法与刺激穴位的外疗法相融合,内外并举法治疗双心疾病疗效颇佳。With the accelerated pace of society and increased stress,cardiovascular diseases and mental disorders,such as depression and anxiety disorders,often occur in the same patient at the same time,a phenomenon known as"double heart disease".Cardiovascular disease has been the leading cause of death worldwide,while mental health problems continue to place a heavy economic and social burden on individuals and society.Previous research has focused on the effects of cardiovascular disease or mental health problems alone,with less attention paid to the co-existence of the two,which has led to a failure to consider the full range of cardiac and psychosocial needs when treating bipolar disease,and thus affecting the effectiveness of treatment.ZHANG Yan believes that the pathogenesis of double heart disease is always based on deficiency,with deficiency of qi,blood,Ying and Wei as the root cause and stagnation of qi,phlegm and blood stasis as the symptoms and that if the disease is prolonged for a long period,there will be a mixture of deficiencies and realities.According to ZHANG Yan's many years of experience,based on the theory of"harmonizing Ying and Wei,relieving depression and resolving turbidity",the treatment of Double Heart Disease is based on the understanding of Double Heart Disease from the perspective of heart,Ying and Wei,liver,and spleen,and the combination of internal treatment of oral soup and external treatment of stimulating acupoints,which is effective in treating Double Heart Disease from the perspective of internal and external treatments.
