透骨草方治疗脂溢性脱发的作用机制

摘　　要：脂溢性脱发是目前临床上最常见又难以治愈的脱发类疾病,其发病人数众多,辐射人群广,临床表现以头发大面积脱落,产生油腻异味并且伴有大量头屑为主,严重影响了患者的正常工作及生活,故其研究意义重大。中医治疗脂溢性脱发具有优越性,其经验丰富,能显著改善症状,减少复发等。透骨草方为“皮科泰斗”赵炳南先生所创制的治疗脂溢性脱发的经验方,具有清热除湿,解毒祛脂的功效,恰能针对脂溢性脱发“中焦湿热内蕴”的首要病机。文章阐述了脂溢性脱发的中医学认识及其发病与脾胃湿热内蕴的紧密联系,确定了以清热利湿为要务的治疗原则。现代药理学研究发现,君药透骨草中存在的黄体酮成分,能够阻止破坏毛囊的关键物质双氢睾酮合成。同时透骨草方四味主药中的化学成分分别具备抗炎,抗过敏反应,有抑制微生物生长,养发、生发等功效,可以与中医的清热除湿,解毒止痒等概念相互对应,为透骨草方能有效治疗脂溢性脱发提供了强有力的现代理论依据。文章借鉴了透骨草方治疗脂溢性脱发的典型病例,将其深厚的临床治疗经验与现代医学药理研究相互融合,突出了透骨草方在治疗脂溢性脱发方面的显著疗效,为脂溢性脱发的现代化治疗提供了宝贵借鉴和有效启发。Seborrheic alopecia is the most common and difficult to cure hair loss disease in clinical practice,which has a large number of incidences and a wide radiation population,and the clinical manifestations are mainly large hair loss,greasy odor,and a large amount of dandruff,which seriously affect the normal work and life of the patients,so its research is of great significance.Chinese medicine has superiority in treating seborrheic alopecia,which has rich experience and can significantly improve the symptoms and reduce recurrence.The Turbinaria formula is an empirical formula for the treatment of seborrheic alopecia created by Mr.ZHAO Bingnan,a leading dermatologist,which has the efficacy of clearing heat and removing dampness,detoxifying and eliminating fat,and it can precisely address the primary pathogenesis of seborrheic alopecia,which is the‘dampness and heat in the middle Jiao’.This article explains the understanding of Chinese medicine of seborrheic alopecia and its close connection with the internalization of dampness-heat in the spleen and stomach and defines the treatment principle of clearing heat and removing dampness as the first priority.Modern pharmacological research has found that the presence of progesterone in the monarch drug Turbinaria can prevent the synthesis of dihydrotestosterone,a key substance that destroys hair follicles.At the same time,the chemical components in the four main medicines of the Turbinaria formula have anti-inflammatory,anti-allergic reactions,inhibit the growth of microorganisms,nourish and regenerate hair,etc.,which can correspond to the concepts of Chinese medicine,such as clearing heat and removing dampness,detoxifying,and relieving itching,which provide a strong modern theoretical basis for the effective treatment of seborrheic alopecia by the formula of the Turbinaria formula.At the same time,this paper draws on the typical cases of seborrheic alopecia treated by the Turbinaria miltiorrhiza formula and integrates its profound clinical treatment experience with t
