安神汤对双相情感障碍躁狂发作患者的疗效研究

摘　　要：目的 观察安神汤对双相情感障碍躁狂发作患者的治疗效果。方法 选取2022年6月至2023年6月河北省秦皇岛市九龙山医院收治的双相情感障碍躁狂发作患者100例为研究对象,按照随机数字表法分为2组,对照组50例予常规西医治疗,观察组50例在对照组的基础上加用安神汤治疗。2组均治疗6周后统计疗效,比较2组治疗前后神经递质[包括五羟色胺(5-HT)、多巴胺(DA)、γ氨基丁酸(GABA)]、应激反应指标[包括肾上腺素(E)、去甲肾上腺素(NE)、皮质醇(Cor)]、炎症因子[包括肿瘤坏死因子α(TNF-α)、超敏C反应蛋白(hs-CRP)、白细胞介素6(IL-6)]水平变化情况,统计2组患者治疗期间不良反应发生情况。结果 观察组总有效率92.00%(46/50),对照组总有效率74.00%(37/50),观察组总有效率高于对照组(P<0.05)。与本组治疗前比较,2组治疗后神经递质指标5-HT、DA水平均降低(P<0.05),GABA水平均升高(P<0.05),且治疗后观察组5-HT、DA水平低于对照组(P<0.05),GABA水平高于对照组(P<0.05)。与本组治疗前比较,2组治疗后应激反应指标E、NE、Cor水平均升高(P<0.05),但治疗后观察组E、NE、Cor水平低于对照组(P<0.05)。与本组治疗前比较,2组治疗后炎症因子指标TNF-α、hs-CRP、IL-6水平均降低(P<0.05),且治疗后观察组TNF-α、hs-CRP、IL-6水平均低于对照组(P<0.05)。观察组不良反应发生率12.00%(6/50),对照组不良反应发生率为8.00%(4/50),2组不良反应发生率比较差异无统计学意义(P<0.05)。结论 安神汤对双相情感障碍躁狂发作患者有良好的治疗效果,可降低患者应激水平和炎症因子水平,改善神经递质水平,治疗安全性良好。Objective To study the therapeutic effect of Anshen Decoction on manic episode in patients with bipolar disorder(BD).Methods One hundred BD patients with manic episodes visited in Qinhuangdao Jiulongshan Hospital during June 2022 to June 2023 were recruited,they were randomized 1∶1 to control group and observation group.All patients were treated with conventional Western medicine,and the observation group was additionally treated with Anshen Decoction.After 6 weeks of treatment,the key observation was the curative effect,and the changes in neurotransmitters marker including 5-hydroxytryptamine(5-HT),dopamine(DA),gamma-aminobutyric acid(GABA),stress response indexes including epinephrine(E),norepinephrine(NE),cortisol(Cor),inflammatory factors including tumor necrosis factor alpha(TNF-α),high-sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6)before and after treatment between groups.The treatment-emergent adverse events(TEAEs)in the two groups was counted.Results The total effective rate of the observation group was significantly higher than that of control group(92.00%[46/50]vs 74.00%[37/50],P<0.05).After treatment,significantly decreased 5-HT and DA,and significantly increased GABA were found in the both groups(all P<0.05),and the improvements were more pronounced in the observation group(all P<0.05).After treatment,E,NE and Cor in the both groups were significantly increased(P<0.05),but the levels in the observation group were significantly lower than those in the control group(P<0.05).After treatment,TNF-α,hs-CRP and IL-6 in the both groups were significantly decreased(P<0.05),and the decrease was more obvious in the observation group(P<0.05).There was no difference in TEAEs between the observation group and the control group(12.00%[6/50]vs 8.00%[4/50],P>0.05).Conclusion Anshen Decoction has a good therapeutic effect on manic episode in patients with BD,which can reduce the level of stress and inflammatory factors,improve the level of neurotransmitters,with a good treatment safety.
