妊娠糖尿病孕妇胎儿的左心多普勒超声参数价值评估

摘　　要：目的比较妊娠糖尿病(GDM)孕妇与健康孕妇胎儿左心室多普勒超声参数,探讨GDM孕妇胎儿左心影像学参数特点及多普勒超声检测的诊断价值。方法回顾性分析2022年1月至2024年6月于医院产检的124名孕妇的临床资料,根据健康情况将孕妇分为对照组和观察组,每组62名。对照组为健康孕妇,观察组为GDM孕妇,均行多普勒超声检查,比较两组孕妇胎儿的肺静脉血流速度、肺静脉搏动指数、左室心肌做功指数(Tei)以及左心室结构参数。结果观察组肺静脉血流速度低于对照组,胎儿肺静脉搏动指数、Tei均高于对照组,差异有统计学意义(P<0.05)。结论GDM孕妇胎儿的左心多普勒超声参数与健康胎儿有明显区别,多普勒超声能够有效鉴别GDM孕妇胎儿的肺静脉及左心室功能异常情况,能为胎儿早期诊断与干预提供依据。Objective To compare fetal left ventricular Doppler ultrasound parameters between pregnant women with gestational diabetes mellitus and healthy pregnant women,and to investigate the characteristics of foetal left heart imaging parameters in pregnant women with gestational diabetes mellitus and the value of applying Doppler ultrasound detection.Methods The clinical data of 124 pregnant women who underwent obstetric examination in the hospital from January 2022 to June 2024 were retrospectively analysed,and the pregnant women were divided into a control group and an observation group according to their health conditions,with 62 cases in each group.Doppler ultrasound was performed on healthy pregnant women in the control group and pregnant women with gestational diabetes mellitus in the observation group to compare the pulmonary venous flow velocity,pulmonary venous pulsatility index,left ventricular myocardial work index(Tei),and the structural parameters of the left ventricle in the foetuses of the two groups.Results The pulmonary venous blood flow velocity in the observation group was lower than that in the control group(P<0.05),and the fetal pulmonary venous pulsatility index and left ventricular myocardial work index(Tei)in the observation group were higher than those in the control group,with statistically significant differences(P<0.05).Conclusion The left heart Doppler ultrasound parameters of fetuses of pregnant women with gestational diabetes mellitus are significantly different from those of healthy fetuses,and Doppler ultrasound can effectively identify abnormalities of pulmonary vein and left ventricular function in fetuses of pregnant women with gestational diabetes mellitus,which can provide the basis for early foetal diagnosis and intervention.
