摘  要:目的探讨精益六西格玛(LSS)管理在新生儿重症监护室(NICU)设备管理中的应用价值。方法选取2019年1月至2020年12月医院NICU的159台(套)设备为研究对象,于2019年1—12月采用常规管理,于2020年1—12月对同一批设备采用LSS管理,同期医院的60名医护人员参与研究,比较实施LSS管理前后呼吸机和心电监护仪的使用准备时间、仪器管理质量(记录漏登、不良事件、仪器故障、使用中需维修设备、发生故障的次数)、医护人员使用NICU急救仪器情况(环境整理、白班和夜班取用急救设备时间)及对NICU设备相关知识掌握情况(设备理论知识、临床操作掌握)、设备效益管理指标(成本效益增幅、社会效益增幅、设备管理费用增幅、设备开机率)、管理满意度。结果与管理前相比,采用LSS管理后,呼吸机、心电监护仪的使用准备时间短于管理前(P<0.05);记录漏登、仪器故障、使用中需维修设备、平均故障发生次数、不良事件率均低于管理前,仪器表面擦拭消毒合格率高于管理前(P<0.05);环境整理、白班和夜班取用急救设备时间均短于管理前(P<0.05);医务人员对NICU设备理论知识、临床操作掌握评分高于管理前(P<0.05);NICU设备管理成本效益增幅低于管理前,社会效益增幅高于管理前,设备管理费用增幅低于管理前,医疗设备开机率、设备管理满意度高于管理前(P<0.05)。结论LSS管理可以缩短NICU使用仪器准备时间、提高仪器的管理质量、降低相关不良事件的发生,减少护理人员工作量,提高医务人员的设备相关专业水平和使用的满意度,为医院获得更大的经济效益。Objective To explore the application value of Lean Six Sigma in equipment management of neonatal intensive care unit(NICU).Methods A total of 159 NICU devices from January 2019 to December 2020 were selected as the research objects.The conventional management was adopted from January to December 2019,and Lean Six Sigma management was adopted for the same batch of devices from January to December 2020.60 medical staff from the hospital participated in the study during the same period.Compare the use preparation time of ventilator and ECG monitor before and after the implementation of Lean Six Sigma management,the quality of instrument management(record of missed entry,adverse events,instrument failure,equipment needed to be maintained in use,and the number of failures),and the use of NICU emergency equipment by medical personnel(environmental cleaning,and the time to obtain emergency equipment during day and night shift),and the correlation between NICU equipment Knowledge mastery(equipment theoretical knowledge,clinical operation mastery),equipment benefit management indicators(cost benefit increase,social benefit increase,equipment management cost increase,equipment operation probability)and management satisfaction.Results Compared with before management,the preparation time of ventilator and ECG monitor after lean six Sigma management was shorter than before management(P<0.05).The number of record leakage,instrument failure,equipment needing maintenance in use,failure of each instrument and adverse events were all lower than before management,and the pass rate of instrument surface disinfection and wiping was higher than before management(P<0.05).The time of environment cleaning,day shift and night shift fetching first aid equipment were shorter than before management(P<0.05).The scores of medical staff on theoretical knowledge and clinical operation of NICU equipment were higher than those before management(P<0.05).The increase of cost benefit and social benefit of NICU equipment management was higher than tha
