基于表面肌电图探讨火龙罐对脑卒中患者下肢运动功能的影响

摘　　要：目的通过表面肌电图观察火龙罐对脑卒中患者下肢运动功能的影响。方法采用随机对照法将2021年8月至2023年6月于该院针灸康复科住院的64例脑卒中患者分为观察组(32例)和对照组(32例,脱落1例)。对照组予常规护理和康复训练,观察组在对照组治疗基础上实施火龙罐疗法,每次约30 min,每周治疗2次。2组均进行为期4周的治疗。在首次治疗前、治疗4周后对患者进行双侧臀中肌的表面肌电检测,观察治疗前后均方根值(RMS)的变化,在首次治疗前、治疗2周、治疗4周后对患者进行下肢Fugl-Meyer评分(FMA)及Holden步行功能分级评定(FAC)。结果治疗4周后,2组患者不同动作状态下患侧臀中肌RMS均较治疗前明显升高,动作a(健侧负重、患侧迈步)治疗前[(4.26±1.41)μV vs.(4.83±1.71)μV],治疗后[(6.69±0.96)μV vs.(5.71±0.94)μV];动作b(患侧负重、健侧迈步)治疗前[(6.50±1.27)μV vs.(6.46±1.52)μV],治疗后[(7.69±1.39)μV vs.(6.96±1.25)μV],差异均有统计学意义(P<0.05)。2组患者FMA、FAC评分均较治疗前升高,且观察组高于对照组,差异有统计学意义(P<0.05)。结论火龙罐能有效激活患侧臀肌,改善脑卒中患者下肢运动功能、步行能力。Objective To explore the effect of fire dragon jar on lower limb motor function in stroke patients by surface electromyography.Methods A total of 64 stroke patients who were hospitalized in the Department of Acupuncture and Rehabilitation of Quanzhou Hospital of Traditional Chinese Medicine from August 2021 to June 2023 were divided into the observation group(32 cases)and the control group(32 cases,one case of dropout)by randomized controlled method.The control group was given routine nursing and rehabilitation training,and the observation group was treated with fire dragon jar therapy on the basis of the control group,about 30 min each time,twice a week.Both groups were treated for four weeks.Before the first treatment and after four weeks of treatment,the surface electromyography of bilateral gluteus medius muscle was detected,and the changes of root mean square(RMS)before and after treatment were observed.Before the first treatment,after two weeks and four weeks of treatment,the patients were evaluated by Fugl-Meyer score(FMA)and Holden walking function grading(FAC).Results After 4 weeks of treatment,RMS of the gluteus medius on the affected side was significantly higher in both groups than before treatment under different movement states,and before treatment under movement a(bearing weight on the healthy side,stepping on the affected side)[(4.26±1.41)μV vs.(4.83±1.71)μV].After treatment[(6.69±0.96)μV vs.(5.71±0.94)μV];Action b(lateral load,the contralateral stepped)treatment before[(6.50±1.27)μV vs.(6.46±1.52)μV],after treatment[(7.69±1.39)μV vs.(6.96±1.25)μV],the differences were statistically significant(P<0.05).Conclusion Fire dragon jar can effectively activate the affected gluteal muscle and improve the lower limb motor function and walking ability of stroke patients.
