自拟清热活血敛疮方熏洗治疗混合痔术后慢性难愈性创面效果观察

摘　　要：目的观察自拟清热活血敛疮方熏洗在混合痔术后慢性难愈性创面中的临床应用效果。方法回顾性分析2022年3月至2024年6月于南充市中医医院就诊的283例混合痔术后出现慢性难愈合创面患者的临床诊治资料,根据治疗方法不同,将142例采用自拟清热活血敛疮方熏洗治疗者纳入观察组,141例采用高锰酸钾坐浴治疗者纳入对照组。比较两组患者的临床治疗效果、创面愈合情况(愈合时间、渗出液消失时间、水肿消失时间、住院时间)和治疗前后的血清炎症因子水平[白细胞介素-6(IL-6)、白细胞介素-1β(IL-1β)、超敏C反应蛋白(hs-CRP)],同时比较治疗过程中的不良反应发生情况。结果治疗后,观察组患者的总有效率为96.48%,明显高于对照组的88.65%,差异具有统计学意义(P<0.05);治疗后,观察组患者的创面愈合时间、渗出液消失时间、水肿消失时间和住院时间明显短于对照组,差异均有统计学意义(P<0.05);治疗后,观察组患者的IL-6含量、IL-1β和CRP水平分别为(10.32±4.11)ng/L、(8.22±3.03)ng/L,(19.50±5.72)ng/L,明显低于对照组的(18.19±6.52)ng/L、(13.15±2.01)ng/L、(26.51±6.02)ng/L,差异均有统计学意义(P<0.05);治疗过程中,观察组和对照组患者的不良反应总发生率分别为11.27%、12.06%,差异无统计学意义(P>0.05)。结论自拟清热活血敛疮方熏洗能有效促进混合痔术后慢性难愈合创面的修复,缩短治疗时间,减缓炎症反应,且无严重不良反应,具有临床应用价值。Objective To observe the clinical application effect of self-made Qingre Huoxue Lianchuang prescription fumigation in the treatment of chronic hard-to-heal wounds after mixed haemorrhoids surgery.Methods Clinical diagnosis and treatment data of 283 patients with chronic difficult-to-heal wounds after mixed haemorrhoids surgery who attended Nanchong Hospital of Traditional Chinese Medicine were retrospectively analyzed from March 2022 to June 2024.According to different treatment methods,142 patients who were treated with self-made Qingre Huoxue Lianchuang prescription fumigation were included in the observation group,and 141 patients who were given potassium permanganate sitz bath were enrolled as the control group.The two groups were compared in terms of clinical therapeutic effect,wound healing conditions(healing time,disappearance time of exudate,disappearance time of oedema,length of hospital stay),levels of serum inflammatory factors(interleukin-6[IL-6],interleukin-1β[IL-1β],hypersensitive C-reactive protein[hs-CRP])before and after treatment and occurrence of adverse reactions during treatment.Results After treatment,the effective rate in the observation group was 96.48%,which was significantly higher than 88.65%in the control group(P<0.05).The wound healing time,disappearance time of exudate,disappearance time of oedema,and length of hospital stay in the observation group after treatment were significantly shorter than those in the control group(P<0.05).After treatment,the levels of IL-6,IL-1β,and CRP were(10.32±4.11)ng/L,(8.22±3.03)ng/L,and(19.50±5.72)ng/L,significantly lower than(18.19±6.52)ng/L,(13.15±2.01)ng/L,and(26.51±6.02)ng/L in the control group(P<0.05).During treatment,the total incidence rates of adverse reactions in the observation group and the control group were 11.27%and 12.06%,respectively(P>0.05).Conclusion Self-made Qingre Huoxue Lianchuang prescription fumigation can effectively promote the repair of chronic hard-to-heal wounds after mixed haemorrhoids surgery,shorten the treatm
