铜砭刮痧法治疗风寒湿型肩周炎的临床观察

摘　　要：目的观察在国医大师孙申田教授痛证选穴规律指导下铜砭刮痧疗法对风寒湿型肩周炎的效果。方法将70例风寒湿型肩周炎患者用随机数字表法分为对照组与观察组各35例。两组均采用常规护理,在此基础上,对照组采取常规铜砭刮痧法,观察组采用孙申田痛证选穴规律的铜砭刮痧法,均治疗4周。观察两组患者治疗前后临床疗效、视觉模拟疼痛量表(VAS)评分、肩关节功能(Constant-Murley肩关节功能评分)和中医症状评分。结果观察组总有效率为91.43%,显著高于对照组的82.86%(P<0.05);治疗后两组的VAS评分和中医症状积分均较治疗前低,且观察组低于对照组(P<0.05);两组Constant-Murley肩关节功能评分显著高于治疗前,且观察组高于对照组(P<0.05)。结论运用孙申田教授痛证选穴规律制定的铜砭刮痧法治疗风寒湿型肩周炎患者临床疗效好,能有效缓解患者肩部疼痛,改善肩关节活动,提高患者的生活质量。Objective:To observe the effectiveness of copper spatula scraping therapy for wind-cold-dampness type shoulder periarthritis based on the acupoint selection rules established by TCM master Sun Shentian.Methods:70 patients with wind-cold-dampness type shoulder periarthritis were randomly divided into a control group(n=35)and an observation group(n=35).Both groups received routine care.On this basis,the control group underwent conventional copper spatula scraping therapy,while the observation group received copper spatula scraping therapy guided by Sun Shentian′s acupoint selection rules for pain syndromes.All patients were treated for 4weeks.The clinical efficacy,visual analog scale(VAS)scores for pain,shoulder joint function(Constant-Murley score),and TCM symptom scores were evaluated before and after treatment.Results:The total effective rate in the observation group was 91.43%,which was significantly higher than 82.86%in the control group(P<0.05).After treatment,the VAS scores and TCM symptom scores were lower compared with before treatment in both groups,with the observation group showing significantly lower scores than the control group(P<0.05).The Constant-Murley shoulder joint function scores were significantly higher after treatment in both groups,and the observation group had higher scores than the control group(P<0.05).Conclusion:The copper spatula scraping therapy based on Sun Shentian′acupoint selection rules for pain syndromes is clinically effective for patients with wind-cold-dampness type shoulder periarthritis.It can effectively alleviate shoulder pain,improve shoulder joint mobility,and enhance the quality of life for patients.
