摘 要:目的 探究等速-平衡-减重连续性康复训练对脑卒中后偏瘫患者下肢康复及生活质量的影响。方法 选取2021年2月至2022年5月在首都医科大学附属北京康复医院诊治的脑卒中后偏瘫患者114例为研究对象。按照随机数字表法均分为2组,对照组57例患者接受常规康复训练,观察组57例患者接受等速-平衡-减重连续性康复训练。比较2组患者下肢运动功能、步行功能、平衡功能、跌倒效能、生活质量及依从性。结果 训练12周后,2组Fugl-Meyer下肢运动功能评定量表(FMA-LE)评分、6 min内步行距离(6MWT)、Holden步行功能分级、Berg平衡量表(BBS)评分、修订版跌倒效能量表(MFES)评分及健康状况调查简表(SF-36)评分较训练前升高,起立行走计时(TUG)较训练前缩短(P<0.05);且观察组FMA-LE评分、6MWT、Holden步行功能分级、BBS评分、MFES评分及SF-36评分中生理功能、身体活动及一般健康状况评分均高于对照组(P<0.05)。观察组与对照组康复训练依从率的比较,差异无统计学意义(84.21%比78.95%,P>0.05)。结论 等速-平衡-减重连续性康复训练可有效改善脑卒中后偏瘫患者下肢运动功能和平衡功能,提高生活质量。Objective To explore the effects of isokinetic-balance-weight loss continuous rehabilitation training on lower limb rehabilitation and quality of daily life in patients with post-stroke hemiplegia.Methods A total of 114 patients with post-stroke hemiplegia treated in Beijing Rehabilitation Hospital Affiliated to Capital Medical University were enrolled as the research objects from February 2021 to May 2022.They were randomly divided into control group(57 cases,routine rehabilitation training)and observation group(57 cases,isokinetic-balance-weight loss continuous rehabilitation training).The motor function of lower limb,walking function,balance function,falling efficiency,quality of life and compliance were compared between the two groups.Results After training,score of Fugl-Meyer Assessment-Lower Extremity(FMA-LE),6 minutes walking distance(6MWT),grading of Holden walking function,scores of Berg Balance Scale(BBS),Modified Fall Efficacy Scale(MFES)and the 36-item shot-form health status survey(SF-36)were increased,and timed up and go test(TUG)was shortened in both groups(P<0.05).FMA-LE score,6MWT,grading of Holden walking function,scores of BBS,MFES and SF-36(physiological function,physical activity,general health)in observation group were higher than those in control group(P<0.05).There was no significant difference in compliance rate of rehabilitation training between observation group and control group(84.21%vs 78.95%,P>0.05).Conclusions Isokinetic-balance-weight loss continuous rehabilitation training can effectively improve motor function of lower limb,balance function and quality of daily life in patients with post-stroke hemiplegia.
