摘 要:目的研究基质金属蛋白酶(MMP-2、MMP-9)在系统性红斑狼疮(SLE)狼疮性肾炎(LN)患者血清中的表达及其与病情活动的相关性,并分析SLE LN活动患者外周血淋巴细胞亚群的变化与LN进展的潜在相关性。方法①选取2021年6月至2023年6月期间收治的51例SLE LN患者[根据系统性红斑狼疮疾病活动指数(SLEDAI)评分分为活动组(SLEDAI≥10分)和非活动组(SLEDAI<10分)],以及45例健康者作为对照组;②收集并检测3组受试者的血清MMP-2、MMP-9水平;③检测SLE LN活动组患者的ANA、抗dsDNA、hs-CRP、IgA、IgG、IgM、补体C3和C4的表达量;④分析MMP-2、MMP-9与ANA、抗dsDNA、hs-CRP、IgA、IgG、IgM、补体C3和C4等病情活动性指标的相关性;⑤使用流式细胞仪检测SLE LN活动组患者的外周血淋巴细胞亚群。结果①SLE LN活动组患者血清中MMP-2水平显著高于非活动组和对照组(P<0.05),而MMP-9水平显著低于非活动组和对照组(P<0.05);②SLE LN活动组血清MMP-2与ANA、dsDNA、hs-CRP、IgG呈正相关,与补体C3、C4呈负相关;MMP-9与ANA、dsDNA、IgG呈负相关,与补体C4呈正相关(P<0.05);③MMP-9与T细胞百分比呈正相关(P<0.05)。结论MMP-2、MMP-9在SLE LN活动患者血清中的表达与病情活动性密切相关,可能作为早期诊断及评估病情活动的生物标志物。Objective To investigate the expression of matrix metalloproteinases(MMP-2 and MMP-9)in the serum of patients with lupus nephritis(LN)secondary to systemic lupus erythematosus(SLE)and its correlation with disease activity,and to analyze the potential correlation between changes in peripheral blood lymphocyte subsets in patients with active SLE LN and the progression of LN.Methods①Fifty-one patients with SLE LN admitted between June 2021 and June 2023 were enrolled and divided into an active group(SLEDAI≥10)and an inactive group(SLEDAI<10)based on the Systemic Lupus Erythematosus Disease Activity Index(SLEDAI)score,and 45 healthy individuals serving as a control group.②Serum levels of MMP-2 and MMP-9 were collected and measured in all three groups.③Expression levels of ANA,anti-dsDNA,hs-CRP,IgA,IgG,IgM,and complement C3 and C4 were measured in active SLE LN patients.④Correlations between MMP-2,MMP-9,and the aforementioned disease activity markers were analyzed.⑤Peripheral blood lymphocyte subsets were assessed using flow cytometry in active SLE LN patients.Results①Serum MMP-2 levels were significantly higher in the active SLE LN group compared to the inactive group and the control group(P<0.05),while MMP-9 levels were significantly lower(P<0.05).②In the active SLE LN group,serum MMP-2 was positively correlated with ANA,dsDNA,hs-CRP,and IgG,and negatively correlated with complement C3 and C4;MMP-9 was negatively correlated with ANA,dsDNA,and IgG,and positively correlated with complement C4(P<0.05).③MMP-9 was positively correlated with the percentage of T cells(P<0.05).Conclusion The expression of MMP-2 and MMP-9 in the serum of active SLE LN patients is closely associated with disease activity and may serve as biomarkers for early diagnosis and assessment of disease activity.
