清金化痰汤联合孟鲁司特钠治疗小儿支原体肺炎临床疗效及对淋巴细胞、C反应蛋白、LCR的影响

摘　　要：目的观察清金化痰汤联合孟鲁司特钠治疗小儿支原体肺炎临床疗效及对淋巴细胞、C反应蛋白(CRP)、乳酸清除率(LCR)的影响。方法将我院150例支原体肺炎患儿随机分为对照组与实验组各75例,对照组予以孟鲁司特钠治疗,实验组在对照组的治疗基础上联合清金化痰汤治疗,观察两组临床疗效、中医主症状积分、T淋巴细胞水平(CD4^(+)、CD8^(+)和CD4^(+)/CD8^(+))、CRP水平及LCR。结果治疗后,实验组总有效率高于对照组(P<0.05);两组痰壅、咳嗽和发热评分均低于治疗前,且实验组上述指标均低于对照组(P<0.05);两组CD4^(+)、CD4^(+)/CD8^(+)水平均高于治疗前且实验组上述指标均高于对照组(P<0.05)两组CD8^(+)水平均低于治疗前且实验组CD8^(+)水平低于对照组(P<0.05);两组CRP水平均低于治疗前且实验组CRP水平明显低于对照组(P<0.05);治疗24h后实验组LCR显著高于对照组(P<0.05);两组不良反应总发生率比较无显著差异(P>0.05)。结论清金化痰汤联合孟鲁司特钠治疗小儿支原体肺炎疗效较好,可有效缓解患儿临床症状减轻机体的炎症反应。Objective To observe the curative effect of Qingjin Huatan Decoction combined with montelukast sodium and its influences on lymphocytes,C-reactive protein(CRP)and lactate clearance rate(LCR)in children with mycoplasma pneumonia.Methods A total of 150 children with mycoplasma pneumonia were randomly divided into control group(n=75,montelukast sodium)and research group(n=75,Qingjin Huatan Decoction combined with montelukast sodium).The clinical curative effect,scores of main TCM symptoms,T lymphocytes(CD4^(+),CD8^(+),and CD4^(+)/CD8^(+)),CRP and LCR in the two groups were observed and compared.Results After treatment,the total effective rate of research group was higher than that of control group(P<0.05);scores of phlegm obstruction,cough and fever in both groups were decreased,which were lower in research group than control group(P<0.05);CD4^(+)and CD4^(+)/CD8^(+)in both groups were increased,which were higher in research group than control group(P<0.05),while CD8^(+)was decreased in both groups,which was lower in research group than control group(P<0.05);CRP in both groups was decreased,which was lower in research group than control group(P<0.05);after 24 h of treatment,LCR in research group was significantly higher than that in control group(P<0.05);there was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Qingjin Huatan Decoction combined with montelukast sodium has a good therapeutic effect on pediatric mycoplasma pneumonia,which can effectively relieve clinical symptoms and inflammatory response.
