半夏泻心汤治疗慢性荨麻疹临床疗效及对免疫因子水平的影响

摘　　要：目的观察半夏泻心汤治疗慢性荨麻疹的临床疗效及对血清中相关免疫因子水平的影响。方法将我院慢性荨麻疹患者148例随机分为对照组与治疗组各74例。对照组采用抗组胺药盐酸奥洛他定治疗,治疗组在对照组治疗基础上联合半夏泻心汤治疗,评价两组临床疗效、中医证候积分、荨麻疹活动度评分(UAS)、生活质量指数(DLQI)、外周血CD4^(+)、CD8^(+)T细胞及炎性因子白介素-4(IL-4)、白介素-6(IL-6)、白介素-10(IL-10)、肿瘤坏死因子-α(TNF-α)水平。结果治疗后,治疗组总有效率显著高于对照组(P<0.05);两组中医证候积分,荨麻疹活动度评分和生活质量指数评分均明显低于治疗前(P<0.05),治疗组上述指标低于对照组(P<0.05);两组CD4^(+)T细胞含量高于治疗前,CD8^(+)T细胞含量低于治疗前(P<0.05),治疗组上述指标优于低于对照组(P<0.05);两组血清中炎性因子IL-4、IL-6、IL-10和TNF-α水平低于治疗前,治疗组上述指标低于对照组(P<0.05)。结论半夏泻心汤治疗慢性荨麻疹可显著降低中医证候积分、荨麻疹活动度评分和皮肤病患者生活质量指数,降低血清免疫因子水平,呈现出较好的临床疗效适用于临床进一步推广使用。Objective To observe the clinical efficacy of Banxia Xiexin Decoction in treatment of chronic urticaria and its impact on the levels of related immune factors in serum.Methods A total of 148 patients with chronic urticaria were randomly divided into control group and treatment group,with 74 cases in each group.Control group was treated with Olotadine hydrochloride,while treatment group was treated with Banxia Xiexin Decoction on the basis of control group.The clinical efficacy of the two groups was evaluated;the TCM syndrome score,urticaria activity score(UAS),quality of life index(DLQI),peripheral blood CD4^(+)and CD8^(+)T cells,inflammatory factors interleukin-4(IL-4),IL-6,IL-10,and tumor necrosis factor-α(TNF-α)levels before and after treatment were compared between the two groups.Results After treatment,the total effective rate of treatment group was higher than that of control group(P<0.05);TCM symptom scores,urticaria activity score,and quality of life index score were significantly lower than before treatment in both groups(P<0.05),which were lower in treatment group than those in control group(P<0.05);CD4^(+)T cell content in the peripheral blood was higher than before treatment in both groups,while CD8^(+)T cell content was lower(P<0.05),and the above indicators were superior in treatment group to those in control group(P<0.05);the levels of IL-4,IL-6,IL-10,and TNF-αin the serum were lower than before treatment in both groups,which were lower in treatment group than those in control group(P<0.05).Conclusion Banxia Xiexin Decoction can significantly reduce TCM syndrome score,urticaria activity score,and quality of life index of skin disease patients in treatment of chronic urticaria,reduce serum immune factor levels,and shows good clinical efficacy.It is suitable for further clinical promotion and use.
