自拟益阴活血汤联合枸橼酸氯米芬片治疗卵巢储备功能减退的效果及对性激素水平的影响

摘　　要：目的探讨自拟益阴活血汤治疗卵巢储备功能减退的效果及对患者性激素水平的影响。方法回顾性分析2021年6月至2023年6月厦门医学院附属第二医院诊治的80例卵巢储备功能减退患者的临床资料,按照治疗方案将其分为参照组(40例)和研究组(40例),参照组患者采取枸橼酸氯米芬片促排治疗,研究组患者采取枸橼酸氯米芬片促排治疗联合自拟益阴活血汤治疗。比较两组患者的性激素指标、子宫卵巢彩超检查结果、子宫内膜厚度、子宫动脉血流指标水平及治疗有效率。结果研究组患者治疗后的卵泡刺激素(FSH)、卵泡刺激素/黄体生成素(FSH/LH)低于参照组,抗苗勒氏管激素(AMH)、雌二醇(E_(2))值高于参照组,卵巢直径、子宫内膜厚度、窦卵泡计数(AFC)值高于参照组,子宫阻力指数(RI)、子宫动脉搏动指数(PI)、收缩期峰值流速/舒张期流速(S/D)值、中医证候积分水平低于参照组,收缩期峰值(PSV)值高于参照组,治疗总有效率高于参照组,差异有统计学意义(P<0.05)。两组患者治疗后的黄体生成素(LH)值比较,差异无统计学意义(P>0.05)。结论自拟益阴活血汤治疗卵巢储备功能减退,可改善患者性激素指标、子宫卵巢彩超检查结果、子宫动脉血流指标水平,并提高疾病治疗总有效率,效果显著。Objective To investigate the effect of Self-made Yiyin Huoxue Decoction on diminished ovarian reserve and its effect on sex hormone levels.Methods The clinical data of 80 patients with diminished ovarian reserve diagnosed and treated in the Second Affiliated Hospital of Xiamen Medical College from June 2021 to June 2023 were retrospectively analyzed.According to the treatment plan,they were divided into reference group(40 cases)and study group(40 cases).The patients in the reference group were treated with Clomifene Citrate Tablets,and the patients in the study group were treated with Clomifene Citrate Tablets Combined with Self-made Yiyin Huoxue Decoction.The sex hormone index,uterine and ovarian color Doppler ultrasound examination results,endometrial thickness,uterine artery blood flow index level and treatment efficiency were compared between the two groups.Results After treatment,the levels of follicle stimulating hormone(FSH),follicle stimulating hormone/luteinizing hormone(FSH/LH)in the study group were lower than those in the reference group,the values of anti-mullerian hormone(AMH)and estradiol(E_(2))in the study group were higher than those in the reference group,and the values of ovarian diameter,endometrial thickness and diminished ovarian reserve(AFC)in the study group were higher than those in the reference group,the value of uterine resistance index(RI),uterine artery pulsation index(PI),peak systolic flow rate/diastolic flow rate(S/D)and traditional Chinese medicine syndrome score were lower than those of the reference group,the value of peak systolic flow rate(PSV)was higher than that of the reference group,and the total effective rate was higher than that of the reference group,with statistical significances(P<0.05).There was no significant difference in luteinizing hormone(LH)levels between the two groups after treatment(P>0.05).Conclusion Self-made Yiyin Huoxue Decoction for the treatment of diminished ovarian reserve can improve the sex hormone indexes,uterine and ovarian color ultrasonic exa
