“调中复衡”视域下的胃癌术后调治思路

摘　　要：运用“调中复衡”思想探讨胃癌术后中医药调治思路,认为中焦脾胃减损为始、清浊升降失序为基、炼营化赤难成为辅、正虚邪实冗杂为要。旨在调和中焦气血,纠调偏失,恢复中焦稳态,具体宜采用纳化相协、复运枢机、调养气血、补虚泻实等法,运用中药复方个体化进行相应治疗。治疗之重在于调补脾胃,兹复中焦气血斡旋舒达之态,同时注意祛邪而不损正,清热而不伤阳,理气而不破血。临证时灵活应用调衡之道,施以调中之法,辨病与辨证相结合,并权衡用药的时机与力度,达到促进术后消化功能恢复、抑制胃癌术后进展的目的。The idea of“tiaozhong fuheng”is used to explore the idea of postoperative traditional Chinese medicine regulation and treatment of gastric cancer,it is considered that spleen and stomach in middle jiao impairment is the beginning,disorder of clearing turbidiousness is the basis,refining and transforming the red difficulty into assistance,the principle of positive deficiency and evil excess is the key.The aim is to harmonize qi and blood of middle jiao,correct imbalance of regulation,restore stability of middle jiao,specifically,it is recommended to use methods such as combination of absorption and transformation,restoration of cardinals,regulation of qi and blood,reinforce insufficiency and reduce excessiveness and so on,individualized treatment with traditional Chinese medicine compound prescription should be applied.The key to treatment lies in regulating and tonifying spleen and stomach,restoring the state of harmonizing qi and blood in middle jiao,while paying attention to eliminating evil without damaging vital qi,clearing heat without harming yang,regulating qi without breaking blood.Flexibly applying method of adjusting balance,apply method of adjustment in middle during clinical practice,combining disease differentiation with syndrome differentiation,balancing timing and intensity of medication to achieve the goal of promoting postoperative digestive function recovery,inhibiting postoperative progression of gastric cancer.
