电针治疗膀胱过度活动症的取穴规律及相关参数研究

摘　　要：目的探讨电针治疗膀胱过度活动症(OAB)的取穴特点和相关参数。方法检索中国知网、中国学术期刊数据库、维普网、中国生物医学文献数据库、PubMed、EMbase、Web of Science核心合集数据库2005年1月至2024年6月收录的电针治疗OAB相关研究文献,建立处方数据库。采用SPSS Statistics 27.0及SPSS Modeler 18.0软件分别进行聚类和关联分析。结果纳入41篇文献,共49个电针处方,41个电针方案,腧穴34个,总频次为266,肾俞、次髎、三阴交使用频次较高;常用腧穴有效分布于足太阳膀胱经、任脉、足太阴脾经;最常使用的电针参数是连续波、耐受、50~100 Hz、30 min、1次/d、4周;关联分析支持度最高的穴位配伍为三阴交-肾俞-次髎,聚类分析得出2个核心集群。结论电针是OAB的主要治疗手段,具有良好的临床效果,多使用肾俞、次髎、三阴交等腧穴,电针参数常选择连续波、耐受、50~100 Hz、30 min、1次/d、4周,以期为电针治疗OAB的临床治疗提供思路。Objective To investigate point selection characteristics and related parameters of electroacupuncture in the treatment of overactive bladder (OAB). Methods Research literatures related to electroacupuncture in the treatment of OAB in CNKI, CSPD, VIP, CBM, PubMed, EMbase, Web of Science Core Collection Database from January 2005 to June 2024 were searched. A prescription database was established. Clustering and association analysis were performed by using SPSS Statistics 27.0 and SPSS Modeler 18.0 software, respectively. Results A total of 41 literatures were included, included 49 electroacupuncture prescriptions, 41 electroacupuncture schemes and 34 acupoints, with a total frequency of 266 times. The frequency of shènshù, cìliáo, sānyīnjiāo were high. The commonly used acupoints were effectively distributed in bladder channel of foot taiyang, conception channel, spleen channel of foot taiyin. The most commonly used electroacupuncture parameters were continuous wave, tolerance, 50-100 Hz, 30 min, once a day, and four weeks. The compatibility of acupoints with the highest support in association analysis was sānyīnjiāo-shènshù-cìliáo, and two core clusters were obtained by cluster analysis. Conclusion Electroacupuncture is the main treatment method of OAB, and has good clinical effects. It is commonly used at acupoints such as shènshù, cìliáo, and sānyīnjiāo. Electroacupuncture parameters are often selected as continuous wave, tolerant, 50-100 Hz, 30 min, once a day, for four weeks, in order to provide ideas for clinical treatment of OAB with electroacupuncture.
