居家失能老人社区低价值护理感知体验的质性研究

摘　　要：目的探讨居家照护的失能老人对社区低价值护理的感知体验。方法采用目的抽样法,于2024年2月至5月选取上海市某社区卫生服务中心辖区内13例居家照护的中、重度失能老人进行半结构式访谈,采用质性研究定向内容分析法依据居家养老服务供需适配理论为框架析出研究主题。结果居家失能老人对社区低价值护理的感知体验可归纳为3个主题:社区护理质量信任度较低、护理服务可传递性局限程度大、症状控制有效性体现不足。结论居家照护失能老人对社区护理支持价值的认同感处于较低水平,且影响因素复杂,社区护理人员应当提升护理职业责任感,在社区护理工作中充分考虑服务对象特质,给予闭环式、专业性的护理服务,提高护理质量及效率。Objective To explore the perceived experience of low-value care in community for elderly people with disabilities at home.Methods Sampling method was used to conduct semi-structured interviews on 13 elderly persons with moderate to severe disability under home care in a community health service center in Shanghai from February to May 2024.Qualitative research-oriented content analysis method was adopted to separate the research theme according to the supply-demand adaptation theory of home care service.Results The perception and experience of the elderly with disabilities at home on the low-value care in the community can be summarized into three themes:low trust in the quality of community nursing,large limitation of the transferability of nursing services,and insufficient manifestation of the effectiveness of symptom control.Conclusion The sense of identity of disabled elderly people in home care to the value of community nursing support is at a low level,and the influencing factors are complex.Community nursing staff should enhance the professional responsibility of nursing,fully consider the characteristics of service objects in community nursing work,and provide closed-loop and professional nursing services to improve nursing quality and efficiency.
