摘　　要：【目的】探讨老年脓毒症患者血清降钙素原(PCT)水平、急性生理学与慢性健康状况评价Ⅱ(APACHEⅡ)评分变化及影响因素。【方法】回顾性分析90例老年脓毒症患者的临床资料,根据其APACHEⅡ评分分为高危组(n=24)、中危组(n=46)和低危组(n=20)。比较三组患者PCT水平、APACHEⅡ评分,采用Pearson分析PCT与APACHEⅡ评分的相关性,采用多因素Logistic回归分析影响老年脓毒症患者预后不良的危险因素。【结果】低危组PCT水平、APACHEⅡ评分低于中危组及高危组,且中危组低于高危组(P<0.05)。Pearson检验显示,PCT水平与APACHEⅡ评分呈正相关(r=0.857,P<0.05)。Logistic回归分析结果显示,PCT水平、APACHEⅡ评分升高是老年脓毒症患者预后不良的独立危险因素(OR=5.078、5.043,P<0.05)。【结论】老年脓毒症的发生以及病情进展与血清PCT水平升高、APACHEⅡ评分上升有关,临床应予以重视。【Objective】To investigate the changes of serum procalcitonin(PCT)level,acute physiology and chronic health evaluationⅡ(APACHEⅡ)score and its influencing factors in elderly patients with sepsis.【Methods】Clinical data of 90 elderly patients with sepsis were retrospectively analyzed and divided into high risk group(n=24),middle risk group(n=46)and low risk group(n=20)according to their APACHEⅡscores.PCT level and APACHEⅡscore of the three groups were compared,Pearson test was used to analyze the correlation between PCT and APACHEⅡscore,and multiple Logistic regression was used to analyze the risk factors affecting senile sepsis.【Results】The PCT level and APACHEⅡscore of the low-risk group were lower than those of the medium-risk group and high-risk group,and the medium-risk group was lower than the high-risk group(P<0.05).Pearson test showed that PCT level was positively correlated with APACHEⅡscore(r=0.857,P<0.05).Logistic regression analysis showed that PCT level and APACHEⅡscore were independent risk factors for senile sepsis(OR=5.078,5.043,P<0.05).【Conclusion】The occurrence and progression of senile sepsis are related to the increase of serum PCT level and APACHEⅡscore,which should be paid attention to clinically.
