中药熏洗辅助体外发散式冲击波治疗慢性跖筋膜炎跟痛症患者的疗效

摘　　要：【目的】观察中药熏洗辅助体外发散式冲击波治疗慢性跖筋膜炎跟痛症患者的疗效及对复发情况的影响。【方法】本院收治的94例慢性跖筋膜炎跟痛症患者随机分为对照组和观察组,每组47例。对照组予以发散式体外冲击波治疗,观察组在此基础上加用中药熏洗,两组患者均在医师指导下行跖筋膜及跟腱拉伸锻炼,比较两组患者的临床疗效及复发情况。【结果】治疗后,两组患者视觉模拟评分法(VAS)评分较治疗前均降低(P<0.05),且观察组低于对照组(P<0.05);两组美国矫形外科足踝协会(AOFAS)量表评分较治疗前均升高(P<0.05),且观察组AOFAS量表评分高于对照组(P<0.05);观察组治疗后1年复发率为10.64%,低于对照组的27.66%(P<0.05);治疗后,两组患者中医证候积分较治疗前均降低(P<0.05),且观察组低于对照组(P<0.05)。观察组不良反应总发生率为8.51%,高于对照组的4.26%,但两组比较差异无统计学意义(P>0.05)。【结论】中药熏洗辅助体外发散式冲击波治疗可以缓解慢性跖筋膜炎跟痛症患者疼痛症状,改善患者足踝功能,可减少复发,且安全性较好。【Objective】To observe the efficacy of herbal fumigation combined with extracorporeal radial shock wave therapy in patients with chronic plantar fasciitis induced heel pain,and its impact on recurrence.【Methods】A total of 94 patients with chronic plantar fasciitis and heel pain treated in our hospital were randomly divided into the control group and the observation group,with 47 cases in each group.The control group received radial extracorporeal shock wave therapy,while the observation group received additional herbal fumigation.Both groups performed plantar fascia and Achilles tendon stretching exercises under physician guidance.The clinical efficacy and recurrence rates of the two groups were compared as well.【Results】After treatment,the visual analog scale(VAS)pain scores of both groups decreased compared to before treatment(P<0.05),with the observation group showing lower scores than the control group(P<0.05).The American Orthopaedic Foot and Ankle Society(AOFAS)scale scores of both groups increased compared to before treatment(P<0.05),with the observation group showing higher scores than the control group(P<0.05).The recurrence rate in the observation group one year after treatment was 10.64%,which was lower than that in the control group's 27.66%(P<0.05).After treatment,Traditional Chinese Medicine syndrome scores in both groups decreased compared to before treatment(P<0.05),with the observation group showing lower scores than the control group(P<0.05).The total incidence of adverse reactions in the observation group was 8.51%,which was higher than the control group's 4.26%,but the difference was not statistically significant(P>0.05).【Conclusion】Herbal fumigation combined with extracorporeal radial shock wave therapy can alleviate pain symptoms in patients with chronic plantar fasciitis induced heel pain,improve foot and ankle function,help reduce recurrence,and has good safety.
