摘 要:阻抗是医疗活动中常见的会影响医患沟通和诊疗效果的消极社会行为。该文以真实的医生与老年患者的会话为研究对象,采用会话分析的研究方法,探究医生与老年患者会话中阻抗现象的具体表现形式与阻抗话语的序列结构特征,并结合医生的应对策略,分析阻抗及其应对对医生与老年患者之间权势关系的展现和影响。研究发现,老年患者在医疗活动中表现出沉默、重复、反问、质疑、答非所问、转移话题、异议、反驳、抱怨等形式的阻抗,医生通常以重复提问、耐心解释、积极回应、认可等方式应对阻抗,努力实现医患间权势关系的平衡。Resistance is a socially negative behavior commonly seen in medical interactions that will negatively affect doctor-patient interaction and treatment outcomes.Based on the data from real-life interactions between doctors and elderly patients,this study explores specific manifestations and characteristics of sequence organization of resistance in interactions between doctors and elderly patients from the perspective of conversation analysis.It also pays attention to doctors'coping strategies for resistance to discuss how resistance and corresponding coping strategies reveal and affect the power relations between doctors and patients.It is found that elderly patients in medical interactions manifest various forms of resistance such as silence,repetition,question,query,evasive answer,topic diversion,different opinion,explicit rejection and complaint.In response,doctors typically employ strategies like repeating questions,patiently explaining,actively responding,and acknowledging patients'concerns to address resistance,striving to achieve a balance in the power relation between doctors and patients.
