摘　　要：目的:探讨突发性耳聋患者血清血管细胞黏附分子-1(VCAM-1)、上皮中性粒细胞活化肽-78(ENA-78)、基质金属蛋白酶-2(MMP-2)水平变化及检测意义。方法:选取接受治疗的突发性耳聋患者136例为观察组,另选取同期体检健康者136例为对照组。采用ELISA法测定血清VCAM-1、ENA-78、MMP-2水平。根据听力损失程度将患者分为轻度组(46例)、中度组(51例)和重度组(39例)。根据治疗效果将患者分为疗效良好组(82例)和疗效不良组(54例)。比较观察组和对照组血清VCAM-1、ENA-78、MMP-2水平。比较不同听力损失程度及治疗效果患者血清VCAM-1、ENA-78、MMP-2水平。绘制受试者工作特征(ROC)曲线分析血清VCAM-1、ENA-78、MMP-2对突发性耳聋患者疗效不良的预测价值。结果:观察组血清VCAM-1、ENA-78、MMP-2水平高于对照组(均P<0.05)。与轻度组比较,中度和重度组血清VCAM-1、ENA-78、MMP-2水平逐渐升高(均P<0.05)。疗效良好组和疗效不良组患者临床资料比较差异无统计学意义(均P>0.05)。与疗效良好组比较,疗效不良组患者血清VCAM-1、ENA-78、MMP-2水平升高(均P<0.05)。血清VCAM-1、ENA-78、MMP-2三者联合预测突发性耳聋患者疗效不良的AUC高于血清单独指标预测的AUC(均P<0.05)。结论:突发性耳聋患者血清VCAM-1、ENA-78、MMP-2水平升高,与听力损失程度有关,三者联合对突发性耳聋患者治疗效果的预测价值较高。Objective:To investigate the changes in serum levels of vascular cell adhesion molecule-1(VCAM-1),Epithelial neutrophil activating peptide-78(ENA-78)and matrix metalloproteinase-2(MMP-2)in patients with sudden deafness and the significance of their detection.Methods:A total of 136 patients with sudden deafness treated were selected as the observation group,and 136 healthy individuals from the same period were chosen as the control group.ELISA was used to measure serum levels of VCAM-1,ENA-78,and MMP-2.Patients were divided into mild(46 cases),moderate(51 cases),and severe(39 cases)groups based on the degree of hearing loss.According to the treatment effect,patients were divided into good efficacy group(82 cases)and poor efficacy group(54 cases).Serum levels of VCAM-1,ENA-78 and MMP-2 were compared between the observation and control groups.The levels were also compared among patients with different degrees of hearing loss and treatment effects.The receiver operating characteristic(ROC)curve was plotted to analyze the predictive value of serum VCAM-1,ENA-78,and MMP-2 for poor treatment outcomes in patients with sudden deafness.Results:Serum levels of VCAM-1,ENA-78,and MMP-2 in the observation group were higher than those in the control group(all P<0.05).Compared with the mild group,serum levels of VCAM-1,ENA-78,and MMP-2 gradually increased in the moderate and severe groups(all P<0.05).There was no statistically significant difference in clinical data between patients in the good efficacy group and the poor efficacy group(all P>0.05).Compared with the good efficacy group,serum levels of VCAM-1,ENA-78 and MMP-2 were elevated in the poor efficacy group(all P<0.05).The combined predictive AUC of serum VCAM-1,ENA-78 and MMP-2 for poor treatment outcomes in patients with sudden deafness was higher than that of individual serum markers(all P<0.05).Conclusion:Elevated serum levels of VCAM-1,ENA-78 and MMP-2 in patients with sudden deafness are related to the degree of hearing loss,and their combination has a high predict
