摘 要:目的系统评价虚拟现实技术(VR)对慢性腰背痛(CLBP)患者疼痛的干预效果,比较VR技术与常规锻炼的疗效差异,为将来临床和科研更规范有效地应用VR技术提供数据和理论依据。方法检索PubMed、CNKI等数据库,检索时限为从数据库建立至2023年8月,由2名研究人员分别筛选接受VR训练的随机对照试验的文献,再共同提取文献相关信息,采用Cochrane偏倚风险评估工具评价纳入文献质量,RevMan 5.4.1软件进行Meta分析。结果9篇文献样本量共514例患者,其中VR试验组255例,对照组259例。Meta分析结果显示:VR训练试验组疼痛评分、恐动症评分及功能障碍评分均明显低于对照组,疼痛评分[MD=-1.66,95%CI(-2.47,-0.85),P<0.001],恐动症评分[MD=-7.64,95%CI(-11.05,-4.23),P<0.001],功能障碍评分[SMD=-0.84,95%CI(-1.34,-0.34),P=0.001]。结论VR干预可以明显减轻慢性腰背痛患者的疼痛程度,降低恐动程度,增强腰椎功能。Objective To systematically evaluate the effect of virtual reality(VR)on pain intervention in patients with Chronic Low Back Pain(CLBP)and compare the effect difference between virtual reality technology and routine exercise,so as to provide data support for more standardized and effective application of VR technology in clinical practice in the future.Methods PubMed,CNKI and other databases were searched,the search time was from the establishment of the database to August 2023,two researchers independently screened the literature of VR training randomized controlled trials,evaluated the literature quality and extracted information.Meta-analysis was performed using RevMan5.4.1 software,and the quality of included references was evaluated according to Cochrane bias risk assessment tool.Results A total of 9 literatures were included,including 514 patients,255 in the VR trial group and 259 in the photographic group.The results of meta-analysis showed:VR training for chronic low back pain score was lower than that of control group[MD=-1.66,95%CI(-2.47,-0.85),P<0.001],Kinesiophobia score[MD=-7.64,95%CI(-11.05,-4.23),P<0.001],Dysfunction score[SMD=-0.84,95%CI(-1.34,-0.34),P=0.001].Conclusion VR training is helpful to improve the pain level of patients with chronic low back pain,and it is effective to improve the level of fear and function of patients.
