摘　　要：目的探讨不同止血方式对腹腔镜卵巢囊肿剔除术患者卵巢储备功能的影响。方法选取2019年1月至2021年12月于吉安市中心人民医院行腹腔镜卵巢囊肿剔除术的65例患者作为研究对象,按照随机数字表法分为A组(n=32)与B组(n=33)。A组接受电凝止血,B组采用缝合止血,比较两组卵巢储备功能、性激素、抗苗勒管激素水平、月经紊乱发生率及并发症发生率。结果术后3、6个月,B组窦状卵泡数量均少于A组,患侧卵巢动脉收缩期峰值血流速度均快于A组,差异有统计学意义(P<0.05)。术后3、6个月,B组雌二醇、卵泡刺激激素、黄体生成素水平均低于A组,AMH水平均高于A组,差异有统计学意义(P<0.05)。B组月经紊乱发生率低于A组,差异有统计学意义(P<0.05);两组并发症发生率比较差异无统计学意义。结论与电凝止血相比,腹腔镜卵巢囊肿剔除术术中缝合止血效果理想,可减少卵巢储备功能损伤。Objective To explore the effects of different hemostasis methods on ovarian reserve function in patients undergoing laparoscopic ovarian cyst removal.Methods 65 patients who underwent laparoscopic ovarian cyst removal in Ji'an Central People's Hospital from January 2019 to December 2021 were selected as the study subjects,they were divided into the group A(n=32)and group B(n=33)according to the random number table method.Group A received electrocoagulation hemostasis,and the group B received suture hemostasis,and the ovarian reserve function,sex hormones,anti-Müllerian hormone(AMH)levels,the incidence of menstrual disorders and complications were compared between the two groups.Results At 3 and 6 months after operation,the number of antral follicle count(AFC)in the group B were less than that in the the group A,and peak systolic velocity(PSV)of the affected ovarian artery in the group B were faster than that in the group A,and the differences were statistically significant(P<0.05).At 3 and 6 months after operation,the levels of estradiol(E2),follicle stimulating hormone(FSH)and luteinizing hormone(LH)in the group B were lower than those in the group A,and the AMH level were higher than that in the group A,and the differences were statistically significant(P<0.05).The incidence of menstrual disorder in the group B was lower than that in the group A,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of complications between the two groups.Conclusion Compared with electrocoagulation hemostasis,suture and hemostasis in laparoscopic ovarian cyst removal is ideal and can reduce the damage of ovarian reserve function.
