摘 要:目的探讨振动排痰仪联合俯卧位机械通气在重症吸入性肺炎患者中的应用效果。方法选取2021年1月至2022年5月黑龙江省医院南岗院区收治的40例重症吸入性肺炎患者作为研究对象,根据治疗方式的不同分为观察组与对照组,每组20例。对照组采用振动排痰仪联合常规仰卧位机械通气治疗,观察组采用振动排痰仪联合俯卧位机械通气治疗。比较两组ICU入住时间、医疗费用、炎症因子水平[白细胞介素-18(interleukin-18,IL-18)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素-6(interleukin-6,IL-6)、前白蛋白(prealbumin,PA)]及血气指标[超敏C反应蛋白(hypersensitive C-reactive-protein,hs-CRP)、呼吸频率(respiratory rate,RR)、心率(heart rate,HR)、动脉血氧饱和度(arterial oxygen saturation,SaO2)]。结果观察组ICU入住时间短于对照组,医疗费用少于对照组,差异有统计学意义(P<0.05)。治疗后,观察组IL-6、IL-8、TNF-α、PA水平均低于对照组,差异有统计学意义(P<0.05)。治疗后,观察组hs-CRP水平低于对照组,RR、HR均慢于对照组,SaO2水平高于对照组,差异有统计学意义(P<0.05)。结论振动排痰仪联合俯卧位机械通气应用于重症吸入性肺炎患者效果良好,可改善机体炎症因子、血气指标水平,值得临床推广应用。Objective To explore the application effect of vibration sputum ejection device combined with prone position mechanical ventilation in patients with severe inhalation pneumonia.Methods 40 patients with severe aspiration pneumonia admitted to Heilongjiang Provincial Hospital Nangang District from January 2021 to May 2022 were selected as the study subjects,and they were divided into the observation group and the control group according to different treatment methods,with 20 cases in each group.The control group was treated with vibration sputum ejector device combined with mechanical ventilation in supine position,and the observation group was treated with vibration sputum ejector device combined with mechanical ventilation in prone position.The ICU stay time,medical cost the level of inflammatory factors(interleukin-18[IL-18],tumor necrosis factor-α[TNF-α],interleukin-6[IL-6],prealbumin[PA]),and blood gas index(hypersensitive C-reactive protein[hs-CRP],respiratory rate[RR],heart rate[HR],arterial oxygen saturation[SaO2])were compared between the two groups.Results The ICU stay time in the observation group was shorter than that in the control group,the medical cost in the observation group was less than that in the control group,and the differences were statistically significant(P<0.05).After treatment,the levels of IL-6,IL-8,TNF-αand PA in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the levels of hs-CRP in the observation group was lower than that in the control group,RR and HR in the observation group were slower than those in the control group,the levels of SaO2 in the observation group was higher than that in the control group,and the differences were statistically significant(P<0.05).Conclusion Vibration sputum ejection device combined with prone position mechanical ventilation has a good application effect in patients with severe inhalation pneumonia,and can improve the level of inflammatory factors and blood
