摘 要:目的探索基于损伤控制外科理论(damage control surgery,DCS)的治疗策略在严重胸部创伤合并多发骨折中的应用效果。方法选取2021年2月至2022年10月抚州市第一人民医院收治的72例严重胸部创伤合并多发性骨折患者作为研究对象,随机分为观察组与对照组,各36例。对照组应用常规治疗方案,观察组应用基于DCS的治疗策略。比较两组生理功能恢复时间指标、手术及住院指标、并发症发生情况、抢救成功率及死亡率。结果观察组乳酸清除、体温恢复及凝血功能恢复时间均短于对照组,差异有统计学意义(P<0.05);观察组手术、机械通气、入住ICU及住院时间均短于对照组,手术出血量、术中输血量均少于对照组,差异有统计学意义(P<0.05);观察组并发症发生率低于对照组,差异有统计学意义(P<0.05);观察组抢救成功率高于对照组,死亡率低于对照组,差异有统计学意义(P<0.05)。结论基于DCS的治疗策略用于严重胸部创伤合并多发性骨折,不仅可快速有效改善患者的生理功能,积极预防并发症的发生,还可提升抢救成功率,加快病情康复速度,值得临床推广应用。Objective To explore the effect of the treatment strategy based on damage control surgery(DCS)theory in the treatment of severe thoracic trauma combined with multiple fractures.Methods 72 patients with severe thoracic trauma combined with multiple fractures admitted to First People's Hospital of Fuzhou from February 2021 to October 2022 were selected as the study subjects,and they were randomly divided into the observation group and the control group,with 36 cases each group.The control group applied the conventional treatment plan,and the observation group applied the DCS-based treatment strategy.The physiological function recovery time indexes,operation and hospitalisation indexes,complication occurrence,resuscitation success rate and death rate were compared between the two groups.Results The recovery time of lactic acid clearance,temperature recovery and coagulation function in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05);the operation,mechanical ventilation,ICU admission and hospital stay in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05);the amount of surgical bleeding and intraoperative blood transfusion were less than those in the control group,and the differences were statistically significant(P<0.05);the incidence rate of complications in the observation group was less than that in the control group,and the difference was statistically significant(P<0.05);the complication rate in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05);the rescue success rate in the observation group was higher than that in the control group,and the mortality rate was lower than that in the control group,and the differences were statistically significant(P<0.05).Conclusion DCS-based treatment strategy for severe thoracic trauma combined with multiple fractures can not only rapidly and effectively impro
